The MDGP's 2025 Young Researcher Award was presented at the 27th Autumn Meeting of the Central German Society for Pneumology and Thoracic Surgery (MDGP), held from November 14th to 15th, 2025 in Magdeburg. One of the two awards was given to Dr. Andreas Hoheisel from Leipzig/Freiburg (Breisgau) for a project on alpha-1 antitrypsin deficiency, which we are very pleased about.

Project title: Non-targeted screening for alpha-1 antitrypsin deficiency

A first publication on this topic, co-authored with PD Dr. med. Thomas Köhnlein and Prof. Dr. Daiana Stolz, was already published in European Respiratory Journal 2024 instead of:

This retrospective study examines a non-targeted screening of patients referred to sleep medicine and allergy practices. It investigates the prevalence of alpha-1 antitrypsin deficiency in patients referred for various reasons, even without respiratory symptoms. The aim was to determine whether individuals with this genetic deficiency can remain healthy into old age and do not necessarily develop symptoms.

All participants underwent routine blood tests to determine their alpha-1 antitrypsin levels, regardless of whether they presented with respiratory conditions such as cough or shortness of breath, or with other complaints such as allergies or suspected sleep apnea. A total of 5,325 individuals were examined, approximately half for respiratory conditions and the other half for non-respiratory problems. Forty individuals, less than one percent of those examined, showed significantly reduced alpha-1 antitrypsin levels, indicating a severe genetic deficiency. These patients were between 27 and 88 years old, and the most frequent genetic finding corresponded to a severe deficiency type. Liver function was almost always normal; only one individual developed symptoms before the age of 40. From the age of 40 onward, approximately half of the affected individuals exhibited lung disease, which manifested itself with declining lung function with increasing age. Many affected individuals, however, had no or only mild symptoms, despite having the genetic defect. More than half had smoked at some point in their lives, yet only a portion of them developed lung disease.

The results show that alpha-1 antitrypsin deficiency can manifest very differently and does not automatically lead to illness, even in old age. These are encouraging results that now support our long-held belief that "a deficiency is not necessarily an illness" with data.

Since we were in Berlin together when Dr. Hoheisel received the news, we were able to congratulate him personally and pledge and discuss our cooperation for further Alpha-1 projects.

Gender medicine initially emerged from the women's rights movement and women's health research in the USA in the 1980s. It now occupies an important place in both science and clinical practice and is essential for improving men's and women's health. It investigates biological differences as well as gender-related sociocultural influences on diseases and the experience of illness.

Gender-specific differences in alpha 1-antitrypsin deficiency

Due to the small number of patients in individual countries and centers, it is particularly difficult to investigate the gender-specific aspects of rare diseases, which is why no meaningful data on this topic exist for AATM. Therefore, our aim was to investigate the gender issue using the prospective international registry of the European Alpha-1 Research Collaboration (EARCO) for patients with AATM.

Gender-specific differences in patient characteristics

The gender question was specifically investigated in patients with the PiZZ genotype. Men in this cohort had a higher BMI. Women were more likely to have the disease diagnosed through family screening, whereas they were less likely to present with liver disease.

Gender-specific differences in exposure

Compared to male study participants, women were more likely to be never-smokers or to report a lower total nicotine consumption, measured in pack-years. Exposure to other inhalable substances such as biomass, cocaine, e-cigarettes, or marijuana was rare and showed no significant differences between the sexes. However, occupational exposure did reveal differences between the sexes: men were more frequently exposed to gases, fumes, dust, asbestos, or other substances than women. Differences also existed in alcohol consumption: women reported less active alcohol consumption, and the number of units of alcohol consumed per week was lower.

Sex-specific differences in lung and liver diseases associated with alpha-1 antitrypsin deficiency

Looking at lung diseases associated with alpha-1 antitrypsin deficiency, men were more likely to have COPD, pulmonary emphysema, or chronic bronchitis, while female participants were more likely to have bronchiectasis. Regarding liver disease, male participants were more likely to have liver disease, cirrhosis, fibrosis, or fatty liver.

Gender-specific differences in the COPD subgroup of the EARCO registry

When specifically examining patients with alpha-1 antitrypsin deficiency and COPD, differences in risk behavior are initially apparent: Men reported a higher total nicotine consumption, measured in pack-years. Regarding COPD severity, women with COPD were more frequently classified as GOLD grade 1 (mild COPD) and less frequently as GOLD grade 4 (very severe COPD) than men. Nevertheless, women with COPD exhibited a significantly higher symptom burden. Furthermore, women in this subgroup reported at least one exacerbation in the past year more frequently than men.

Independent risk factors for symptom burden and exacerbation

Additional analyses were conducted to investigate potential influencing factors such as smoking status, COPD, and sex on symptom burden or exacerbation. In these studies, female sex emerged as an independent risk factor for increased symptom burden and the occurrence of at least one exacerbation within the past 12 months.

Conclusion

Gender appears to play a role in alpha-1 antitrypsin deficiency in many ways. Due to differences in risk behavior, gender should be given greater consideration in alcohol and tobacco prevention programs. Differences in lung diseases, such as the prevalence of COPD and emphysema in men and bronchiectasis in women, highlight the need for tailored treatment approaches that take gender into account. It is hoped that further research in this area will lead to a better understanding of the relationships and causes of these differences and to the development of gender-adapted treatment strategies.

An online survey on "Genetic Newborn Screening" is currently underway, which may also be of interest to parents and expectant parents with a family history of alpha-1 deficiency. The survey is supported by both the Achse and the Children's Network, and we also support the project.

The well-known newborn screening is a voluntary blood test performed on infants just a few days old to screen for a number of pre-defined diseases. This survey explores the potential expansion of this test through so-called "genomic" testing after birth. This genomic newborn screening (gNBS) could detect significantly more congenital diseases early on, as it would allow for the simultaneous analysis of a large number of genes.

The survey is part of a research project funded by the BMBF (Federal Ministry of Education and Research). NEW_LIVES, which is conducted at the universities of Heidelberg and Mannheim.

The aim of the project is to assess whether a gNBS (growth-friendly baby school) is a viable option for Germany. In the online survey, the project team is interested in the attitudes and wishes of expectant parents and parents with at least one child under 8 years old. The results of the survey will be incorporated into a project statement.

Target group: Expectant parents or parents with at least one child under 8 years old

Duration: approx. 30 minutes

Compensation: 10 euros per person, or couples can receive a total of 25 euros.

Participation via a laptop/PC is recommended, as the questionnaire's display is not optimized for mobile phone use.

The post Forschungsprojekt „New Lives“ – Neugeborenen-Screening appeared first on Ihr Online Portal für Mitglieder und Interessierte.

At the congress of the German Society for Pneumology (DGP), Alpha1 Germany e. V. conducted an interview with Dr. Christoph Fisser, spokesperson for YoungDGP and senior physician at the University Hospital of Regensburg. The interview focused on key questions: Dr. Fisser's first encounter with alpha-1 antitrypsin deficiency, the importance of rare diseases in medical education, YoungDGP initiatives to inspire students to study pulmonology, and the valuable exchange between pulmonologists and patient organizations.

Dr. Fisser emphasized the role of the younger generation in medical research and innovation. "Young people provide the crucial input because they bring new ideas and often think 'outside the box'."„

These perspectives not only highlight the current challenges, but also the vibrant dynamics in pulmonology, driven by young medical professionals.

Alpha1 Germany would like to thank Dr. Fisser for his commitment and his willingness to give an interview.

Dear Alphas,
As you may already know, recruitment is currently underway for the Phase 3 trial of the drug Fazirsiran.

Fazirsiran is currently the most promising treatment for liver disease associated with alpha-1 antitrypsin deficiency. This drug belongs to the class of siRNAs and reduces the production of the mutated alpha-1 antitrypsin without altering the genes. This is intended to halt the progression of the liver disease and allow for liver regeneration.

The Phase 2 trial has already yielded promising results: Several markers of liver damage improved significantly, and fazirsiran administration led to a reduction of more than 801 TP3T in the mutated protein in both blood and liver tissue. During the one-year observation period, lung function remained stable, and no serious adverse events occurred that would have necessitated discontinuation of the study. If you or your relatives carry the Pi*ZZ genotype, are affected by liver disease as part of alpha-1 antitrypsin deficiency, and do not have advanced lung disease, please contact us by phone (+49 241 80 36606) or email (alpha1@ukaachen.de). We would be happy to advise you on your individual case.
We would be delighted if you would help us bring the first drug for the treatment of alpha1-antitrypsin deficiency-associated liver disease to the clinic!

Greetings from Aachen!

Since 2004, Grifols has been supporting the realization of innovative ideas and visions in the field of Alpha-1 Antitrypsin Deficiency (AATD) through the ALTA AWARD. Each year, the ALTA AWARD provides the recipients with scientific and financial freedom to pursue clinical and scientific research in AATD and AATD-associated diseases, to gather new insights into Alpha-1, and thus to help shape the future of AATD therapy.

Grifols funds two research projects each year with an annual prize of €50,000 per grant. The ALTA AWARD is therefore one of the most prestigious prizes in the field of respiratory research.

The award winners are selected by an independent panel of experts. The 2023 winners are from the USA and France: Dr. Vickram Tejwani (Assistant Professor at Cleveland Clinic Lerner College of Medicine, Ohio) and Dr. Samuel Amintas (Associate Professor at the Bordeaux Institute of Oncology and Bordeaux Hospital).

It is unlikely that anyone diagnosed with Alpha1 antitrypsin deficiency has ever had a pulse oximeter applied, whether as a handheld device barely the size of a baby's fist at the doctor's office or as a simple sensor wired to other monitoring devices in the clinic.

In both cases, the device is clipped onto a finger and then measures – as its name suggests – heart rate (pulse) and oxygen, more precisely: the degree of oxygen saturation of the blood, i.e., how many of the red blood cells are carrying oxygen. The initially quite simple-looking device uses a rather complex measurement method, namely spectral oximetry (i.e., oxygen measurement using light):

On one side of the clip is a light source that emits light in 2 different wavelengths.

On the opposite side is an optical receiver that measures how much light is absorbed by the blood in the finger and calculates the oxygen saturation value, expressed in %SpO₂.2, from the English „saturation peripheral O2“taken over”.

100 % SpO2 This would mean that the blood is saturated with oxygen to the highest possible degree. However, even perfectly healthy people usually don't reach this level in everyday life; their values typically range between 96 and 99.1, Other sources specify 92–97 % as the normal range.2.

These values apply to resting states. During physical exertion, oxygen saturation remains high in healthy individuals; only under extreme stress might a slight decrease occur. In contrast, individuals with significant impairments in lung function, particularly gas exchange (e.g., due to pulmonary emphysema), even moderate exertion leads to a measurable decrease in saturation.

Oxygen saturation can also drop during sleep, especially when lying on your back, which is sometimes not conducive to deep breathing.3, This is because it can intensify snoring, which, in particularly severe cases, can lead to actual pauses in breathing ("obstructive sleep apnea"). In some cases, for example in heart patients, the breathing commands from the respiratory center are also absent ("central sleep apnea"), so that oxygen saturation also drops. As a result of these effects, among others, the oxygen saturation of those affected is usually particularly low in the early morning, but then soon rises again to the usual level.

For people with healthy lungs, a resting saturation level of 92 mg/dL (%) is often considered the lowest acceptable value, and a rapid drop to values below 90 mg/dL (%) is even considered potentially life-threatening. However, in individuals with progressive lung function impairment, adaptation processes occur, which is why a modified perspective may be appropriate: the body can adapt to a lower saturation level to a certain extent.4, It is less the absolute value of oxygen saturation that is important, but rather a noticeable drop. However, due to many individual factors, there can be no universally applicable limit value for people with impaired lung function. Generally speaking, one can assume that an oxygen saturation of below 90 % requires treatment and in the long term at a value of below 85 % critical is4.

In addition to all these factors that make it difficult to make general statements about the individually "correct" oxygen saturation, measurement errors can also occur due to improper use of pulse oximeters, which usually lead to a reading that is too low. The most common causes for this are:

Failure to follow the instructions in the user manual

A device that is too loose or crooked on the finger

Nail polish, artificial fingernails or nail fungus

Cold fingers

Despite all these uncertainties, it is advisable to obtain a pulse oximeter for home use, especially for detecting changes in oxygen saturation in people whose lung function is already impaired, such as many people with alphas.

The following advice can be derived from the experiences of our club members:

Purchase a pulse oximeter for your home medicine cabinet, for example from your local pharmacy or medical supply store; you can also find suitable offers online. Prices range between €20 and €30 (of course, there are also more expensive versions, such as those that store measurement data over time or can even be paired with a smartphone, on which the time course of the measurements can then be displayed – however, users of such sophisticated versions should have a certain affinity for technology and be aware that the added benefit compared to a simpler model is limited).

Measure your oxygen saturation regularly, e.g. once a week (perhaps together with regular blood pressure measurements), and enter the result along with the measured pulse rate in a table.

Take measurements calmly and always at approximately the same time of day.

It is best to start taking measurements after a visit to the doctor, who has confirmed that your current state of health corresponds to your personal normality; this way you can simply take note of the first measurements and do not have to worry immediately if they deviate from what is described as "normal" in relevant magazines or on the internet.

If you then regularly record and track your oxygen saturation in the following weeks, you will see that the result is subject to certain fluctuations and can vary by +/- 2 %; this is due on the one hand to the measurement accuracy of the devices, but also on the other hand to the fact that oxygen saturation in the body is also subject to certain fluctuations.

Regular measurements over a period of several weeks will then lay the foundation for your individual, medium- and long-term oxygen saturation.

If you measure an unusually low value, first check whether the pulse oximeter is correctly positioned on your finger or if other factors could be causing an inaccurate reading; simply repeat the measurement after a rest period of 10 to 15 minutes.

However, if a significant drop in oxygen saturation from the long-term average is confirmed, you should contact your doctor, especially if you notice other signs that indicate a deterioration in your breathing or general health, or if the measured pulse rate has increased significantly compared to the long-term average.

If you experience signs of a decline in your well-being, particularly regarding your breathing, it is advisable to measure your oxygen saturation outside of your usual routine and compare it to your long-term average. If you notice a significant drop, you should contact your doctor.

Monitoring oxygen saturation at shorter intervals is recommended for lung patients if, in addition to chronic symptoms, further acute attacks on the respiratory tract occur, e.g., as part of a cold, a flu-like infection, bronchitis, or a Covid-19 coronavirus infection.

So much for the theory. Figure 1 now shows recordings of the oxygen saturation of a patient with the PI*ZZ profile, whose lungs are damaged by bronchiectasis and emphysema, but whose respiratory pump is still largely intact (for better readability, the manually recorded measurements were transferred to a computer table):

The recordings were started a few days/weeks after a hospital stay, i.e., at a time of symptom-free „normality“.“

The level of the measured values is

Below what is to be expected for a person with healthy lungs.

But significantly higher than what can be expected for those with already severely impaired lung function.

The fluctuations in the measured values within the range specified above are clearly visible.

Due to the frequency of measurements with 93 and 94%, these values can be assumed to be long-term averages in this case.

During the period shown, there is no significant drop in oxygen saturation; this corresponds with the perceptions of the affected individuals, who also did not notice any change in their lung function during the specified period.

Each blackened cell represents a measurement.

It is now quite clear that individual measurements can deviate significantly from the mean value, but overall, during an uncomplicated period, the mean oxygen saturation value is stable and, in the present example, lies between 93 and 94%.

Unfortunately, from the perspective of a concise presentation, but fortunately for those affected, the recordings do not show a clear drop in oxygen saturation, but it is clear that – as already mentioned at the beginning – this would have to be significantly larger than 2% from the long-term average in order to be considered a warning sign.

In summary, it can be stated that:

The pulse oximeter provides patients with lung function problems with an inexpensive device to detect and document a deterioration in lung function.

Important for its application are measurements over a longer, symptom-free period in order to get a feel for its normal level and the usual fluctuations.

A pulse oximeter should be in the home medicine cabinet of every Alpha patient with lung problems and should be used regularly.

Significant drops in measured values should prompt contact with the treating physician, especially if they occur in conjunction with a deterioration of breathing, general well-being, or in the context of another respiratory infection.

For those of you who have now decided to carry out a weekly measurement of your oxygen saturation (and of course also for those who have been doing this for some time), we provide templates of the protocol shown above on our website. download Ready. If you are advised by a doctor to measure your oxygen saturation daily, you can also use this form by applying the scale to the calendar day instead of the calendar week as shown, and using a separate sheet for each month.

Alpha-1 antitrypsin (AAT) is a very important protein in the body and belongs to the acute-phase glycoproteins. AAT regulates inflammation and therefore increases four- to fivefold when the body is fighting infections or inflammation. As a protease inhibitor, AAT suppresses many proteases that can damage lungs and other tissues. Furthermore, AAT boosts the body's defenses independently of protease inhibition due to its wide-ranging immunomodulatory effects.

The gene responsible for AAT production is SERPINA1. It is located on human chromosome 14. In the event of inflammation, this gene ensures that AAT is produced very rapidly, eventually leading to concentrations of more than 1-2 grams of AAT per liter circulating in the blood. An AAT deficiency occurs when individual amino acids in the AAT protein are altered. For example, if glutamic acid at position 264 is replaced by valine, the so-called S variant is formed; if lysine at position 342 is replaced, the Z-deficient variant is formed. The majority of AAT, approximately 70 %, is produced in the liver. In addition, many other tissues and cells contain the SERPINA1 gene. For example, the lungs, kidneys, and skin can also produce AAT.

Early research on AAT

The first protease inhibitors were discovered at the end of the 19th century. In 1894, the Italian researchers Fermi and Pernossi first described "anti-trypsin activity" in the blood. Half a century later, Arne Tiselius in Uppsala, Sweden, developed electrophoresis, a method for separating and differentiating proteins in the blood. In 1955, Herman Schultze from Marburg, Germany, first described an inhibitor of α1-antitrypsin in the α1-globulin protein fraction. The protein AAT itself was isolated three years later.

Researcher CB Laurell from Malmö, Sweden, succeeded in separating the proteins in the blood even more precisely. Using electrophoresis, he examined blood samples from many different patients and discovered that some people with lung diseases lacked the corresponding band in the α1 region. It quickly became clear that this primarily affected members of certain families. This suggested a familial, genetic predisposition. In fact, those affected often had pulmonary emphysema. Laurell and Eriksson first published the connection between AAT deficiency and the development of emphysema 60 years ago, in 1963.

Not only the lungs, but also the liver, are affected by AAT deficiency. Although AAT is produced in liver cells, it does not enter the bloodstream. Instead, it accumulates in the liver cells and is stored as inclusion bodies. Affected patients can develop cirrhosis and liver cancer.

The Pi system (protease inhibitor system), still in use today, was developed in 1970. It distinguishes the variants of AAT according to their ability to move more or less quickly in an electric field. This results in the formation of different bands during isoelectric focusing (a technique for separating different molecules based on differences in their isoelectric point). The classifications are M for medium, S for slow, Z for very slow, and F for fast. Furthermore, the AAT molecules exhibit differences in specific sugar structures, allowing different so-called glycoforms to be associated with health and disease.

Therapy with AAT

After it became clear that some people with emphysema have an AAT deficiency, attempts were made to supply the body with the missing AAT. The first publication on this topic dates back to 1981 by the American research team led by Ronald Crystal (see Prof. Vogelmeier's presentation). The infusion treatment did indeed increase the AAT level in the blood. This replacement therapy has been used for decades in patients with AAT deficiency-related emphysema, provided the patient meets certain criteria.

Recent small studies offer hope that individuals with the skin disease panniculitis could also benefit from AAT infusions. Small reports are being published on people with AAT deficiency who are being treated with AAT therapy for bronchiectasis, asthma, and other conditions. Since AAT increasingly arises in the context of infections and injuries, other applications for AAT therapy are conceivable. However, animal models must first demonstrate the success of AAT application before human trials can begin.

In addition to classic infusion therapy with AAT, treatment via inhalation is also being tested. Another application method, a cream containing 5-10 % AAT applied to the skin, has only been tested in a small study. This option also requires further investigation.

AAT substitution and lung transplantation

When the lungs are severely damaged, people with AAT deficiency can undergo a lung transplant. After the procedure, AAT replacement therapy is discontinued. At the Hanover Transplant Center, patients who had either received regular AAT treatment before surgery or had not previously received AAT treatment were compared. Post-transplant survival differed significantly between these groups, as those without prior replacement therapy lived longer than those who had received AAT infusions for years: After 10 years, 69 of the patients who had not received replacement therapy before the transplant were still alive, compared to only 26 of the transplant recipients who had received prior replacement therapy. The reasons for this surprising result could not be identified in the study. It is possible that the two patient groups differed in certain characteristics and therefore had different prognoses. For those patients who received this therapy before lung transplantation, it may be important to continue replacement therapy after the transplant.

In animal models, mice treated with AAT after lung transplantation showed fewer rejection reactions. This was demonstrated by researchers in Hanover. Some experts are now considering whether patients should continue to receive AAT replacement therapy after lung transplantation. However, there are no good studies on this topic.

News from experimental research

Normal AAT has a specific spatial structure. In people with type Z AAT, the arrangement of the molecules is altered: several molecules clump together to form large "clumps," known as polymers. Such clumps of type Z AAT are found in many, but not all, liver cells. Polymers are also detected in the blood of all patients with type Z AAT. The question is whether or not this presents a clinical problem.

New studies focused on children with AATD. Some of them develop severe liver disease with cirrhosis as infants and even require a liver transplant. In others, newborns experience a blockage of the liver's bile ducts, a condition known as cholestasis, which later resolves. A third group remains free of liver problems throughout their lives. When researchers examined children in a small study, they found, based on liver biopsies, that polymers were present in the livers of all three groups. Therefore, the presence of polymers in the liver alone is not decisive; other environmental and genetic factors must also be present, such as alterations in lipid metabolism.

Several risk factors are known to contribute to the development of liver cirrhosis in adults with AATD over their lifetime. Alcohol consumption and viral liver infections such as hepatitis and diabetes are particularly significant. Modern methods already make it possible to prevent the formation of Z-AAT polymers. For patients with the Z mutation, one of the researchers' ideas was to inhibit AAT synthesis, thus reducing the production of Z-AAT. In a small phase 2 trial, they administered the substance fazirsiran to 16 people with liver disease. The results showed that the accumulation of Z-AAT in the liver and blood decreased significantly under this treatment. This was also confirmed by histological examination of liver biopsies. However, almost all patients experienced adverse effects such as back or chest pain, diarrhea, dizziness, or shortness of breath. The manufacturer has since launched a larger study to better assess the benefits and risks of fazirsiran.

Z-AAT polymers were studied in the blood of people with AATD and a lung disease. Patients with low polymer concentrations had neither better nor worse lung function scores than those with high polymer levels; therefore, there was no correlation with the extent of lung damage. Researchers are continuing to work on polymers to better understand their functions. Finally, considering that the Vikings spread AATD around the world more than 2,000 years ago, one has to wonder why the disease still occurs today. It is possible that AATD not only causes problems but may also have been beneficial to our ancestors under certain circumstances, such as protecting them from contamination by unprocessed foods.

Outlook on clinical trials

In conclusion, the speaker presented a timeline providing an overview of the human clinical trials, as presented at the 8th Patient Congress in Dublin in 2023. Various pharmaceutical companies are currently working on very different topics. New therapies are being tested, such as antiproteases in tablet form, subcutaneous administration of AAT via injection under the skin, and gene therapy. A wide variety of approaches are being used to try to manage AAT deficiency.
