Nutrition – a complex topic. This article focuses primarily on the various components that should be included in the daily diet to optimally nourish the body. A balanced diet, tailored to individual needs, is crucial for supporting individuals with a chronic condition like alpha-1 antitrypsin deficiency! Conversely, malnutrition and nutritional deficiencies can exacerbate the condition, weaken the immune system, and trigger acute exacerbations.

Nutrients

Nutrients are essential substances that the body needs for cell growth, development, and also for providing energy.

Nutrients are divided into macronutrients and micronutrients.

Macronutrients: The three macronutrients are protein, carbohydrates, and fats.

protein It is required for cell and tissue building, as well as for repairs – e.g., in wound healing, etc. – for enzymes and hormones, in which protein acts as a catalyst, i.e., enabling or influencing chemical reactions in the body.

Proteins are classified into essential and non-essential amino acids. Amino acids are organic molecules and thus essentially the building blocks of proteins. Essential means that our bodies need amino acids but cannot produce them themselves; therefore, we must obtain them through our diet.

Protein is found primarily in meat, fish, eggs, but also in legumes, nuts and dairy products.

carbohydrates Carbohydrates are a source of energy—energy we need to live. They are divided into simple and complex forms, which differ in their chemical structure and how they are processed by the body. Simple carbohydrates, also called sugars, consist of short sugar chains and are digested quickly, leading to a rapid rise in blood sugar levels. Complex carbohydrates, such as starch and fiber, have longer sugar chains and are digested more slowly, resulting in a more gradual release of energy. We commonly refer to simple carbohydrates as "bad" and complex carbohydrates as "good." Carbohydrates are found primarily in whole grains, fruits, vegetables, rice, and potatoes.

Fat Fats are important for energy storage, organ protection, and cell growth. They are also essential for the absorption of fat-soluble vitamins (A, D, E, and K). A distinction is made between saturated, unsaturated, and trans fats. Fats are found in vegetable oils, as well as in nuts, seeds, avocados, and fatty fish.

Micronutrients:

Micronutrients These are vital substances that the body needs in small amounts but cannot produce itself. They include vitamins and minerals (including trace elements and macroelements), as well as, depending on the definition, proteinogenic amino acids and essential omega fatty acids. Unlike macronutrients, they do not provide energy, but are essential for metabolic processes and strengthen the immune system.

Vitamins They are important for metabolic processes, strengthen the immune system, and promote and protect cell growth. A deficiency should be avoided, especially with all of the following vitamins.

Vitamin A

Vitamin C

Vitamin E

Vitamin B12

Vitamin D

mineralsThere are two types of minerals: macrominerals and trace elements. Macrominerals are minerals that occur in larger quantities in the body (more than 50 milligrams per kilogram of body weight).

Quantity elementsThe major minerals include calcium, phosphorus, magnesium, sodium, and potassium. Calcium is mentioned here because it performs a variety of functions (building block of bones and teeth, important for the nervous system, muscles, blood clotting, and cell wall stability), but it is also necessary for vitamin D to be absorbed from the gastrointestinal tract into the bloodstream.

However, an overdose/overconcentration of calcium can lead to kidney stones and impaired kidney function.

Trace elements:

Trace elements Trace elements are essential minerals that the body requires only in very small amounts (usually less than 50 mg per kilogram of body weight), but are nevertheless indispensable for a multitude of metabolic processes and functions. They are often components of enzymes and hormones and play an important role in oxygen supply, energy metabolism, and immune responses. Among the most important trace elements are iron, iodine, zinc, and selenium.

iron Iron is important, for example, for the transport and temporary storage of oxygen. It is a component of enzymes whose functions are incredibly diverse: energy production, DNA synthesis, the metabolism of vitamins A, D, and K, amino acids, fatty acids, and cholesterol, the detoxification of substances such as medications, protection against reactive oxygen species, and a role in inflammatory responses.

In addition to iron deficiency, an oversupply with harmful effects is also possible here, such as accumulation in various organs, cell damage and functional disorders.

Dietary fiber

Another component of our diet is dietary fiber. Dietary fiber consists of indigestible, fibrous substances, primarily found in plant-based foods (e.g., fiber in beans, zucchini, and legumes), which pass through the gastrointestinal tract undigested. In the large intestine, they absorb water and swell. The importance of dietary fiber is often underestimated – the name "ballast" may contribute to this. However, dietary fiber has a positive effect on digestion, promotes satiety, and has a prebiotic effect.

Today we know that dietary fiber can even reduce risks such as obesity, high blood pressure, coronary heart disease, diabetes and also colon cancer.

Incidentally, whole grain products with a high proportion of whole grains (e.g., whole rye bread) have a lower glycemic index than products made from milled grains, as do whole grain products made from oats or barley, in contrast to comparable whole wheat products. The low glycemic index means that blood sugar levels rise only slowly.

While there's no such thing as too much fiber in the strictest sense, it can sometimes (especially whole grains) cause gastrointestinal symptoms such as bloating or stomach pain – particularly when you first start increasing your fiber intake. Usually, the body adjusts to the increased fiber intake after a while.

Water Water is essential for the body, as it performs a multitude of functions. It is a key component of all body cells and fluids and serves as a transport medium and solvent for nutrients, hormones, and other vital substances. Water also regulates body temperature, supports metabolic processes, and aids in the elimination of metabolic waste products.

You should consume about 2–3 liters of water per day, depending on age, gender, and activity level. This recommended amount includes all liquids as well as water-rich foods like fruits and vegetables. One-third of your daily water intake is usually obtained through solid food, and more than half through beverages. Mineral water, tap water, and (unsweetened) herbal and fruit teas are preferable. Coffee and black tea are only recommended in moderation (3–4 cups).

Nutrition at AATM

body weight

Generally, body weight should be monitored during AATM (Advanced Adiposity Syndrome). Both being overweight and underweight should be avoided. Body weight is measured using the so-called BMI (Body Mass Index), which indicates the ratio of body weight to height. The BMI is calculated by dividing weight in kilograms by the square of height in meters (kg/m²).

For an AATM, the BMI value should not be below 18.5/19 and likewise should not exceed 25.

Being overweight is particularly bad for liver diseases, but it also puts additional strain on breathing and thus lung function.

Being underweight and malnutrition weaken the immune system and promote acute exacerbations. However, be aware: Malnutrition, including deficiencies in essential nutrients, can also occur in overweight individuals!

composition

An optimal nutritional composition for AATM includes:

20 % Protein

Milk and dairy products daily

Fish 1-2 times per week

Meat/sausage products (45 g per week) and eggs (3 per week) in moderation

30 % Carbohydrates

Variety of nutritional components according to recommendations of the German Nutrition Society

Fruit and vegetables 5 times a day

Sugar and salt in moderation

40 % Fats

Rapeseed, soybean, linseed, hemp and walnut oils are suitable because they contain plenty of omega-3 fatty acids.

as well as nuts such as walnuts, macadamia nuts and cashews

The intake of protein and fats is slightly higher compared to people with healthy lungs.

Helpful tips about food

Meal prep, or in other words: Prepare your meals for several days, portion them, and freeze some of them if necessary. This saves time and effort.

Hygiene in the kitchen

Replace kitchen sponges every 5-7 days – this reduces the bacterial load.

Avoid using harsh cleaning agents that irritate the respiratory tract.

Turn on the extractor fan or ventilate if necessary.

Five to six smaller meals a day are recommended. The sitting position makes eating easier.

Food and liver

To avoid putting additional strain on the liver, the following measures are helpful:

no alcohol

Consume a healthy amount of fruit, as some fruits (e.g., apples, pears, grapes, cherries, figs, mangoes) contain a lot of fructose.

Do not consume foods sweetened with fructose.

adequate protein intake

Insufficient supply of necessary nutrients

If your daily diet does not provide the necessary nutrients, you should definitely talk to your doctor about it.

The following possible measures can then be considered:

Nutritional counseling/nutritional therapy (may be prescribed by a doctor)

Possibly supplemental high-calorie food, especially in cases of underweight.

Vitamin and mineral supplementation should only be done in consultation with a doctor and with appropriate laboratory diagnostics to determine whether a deficiency actually exists.

Optimal body weight

In chronic lung disease, both obesity and malnutrition or weight loss are associated with a poorer prognosis. People with AATD should therefore aim for an optimal BMI between 21 and 23 kg/m². The BMI, or Body Mass Index, is calculated by dividing body weight in kg by height in meters squared. For example, a weight of 64 kg at a height of 1.70 m results in a BMI of 22 kg/m².
The following will primarily focus on underweight and malnutrition.

Causes and consequences of malnutrition

The causes of malnutrition are varied. People with lung diseases have to expend more energy breathing, thus increasing their energy expenditure. Problems such as coughing, sore throat, difficulty swallowing, dental issues, or chewing difficulties impair food intake. Malnutrition is also caused by reduced appetite when one feels tired and exhausted or lacks the energy to eat. A monotonous diet, for example, consisting mainly of bread or rice, and insufficient protein intake, can also lead to malnutrition.
Malnutrition affects not only the body's fat stores but also, and perhaps more importantly, the muscles. When muscle mass is lost, the respiratory muscles and diaphragm are also affected. This restricts breathing and, consequently, lung function. Physical endurance is reduced as a result. Acute flare-ups become more frequent, and the disease progresses more rapidly.
For people with chronic lung disease, it is therefore important to prevent malnutrition and weight loss. This requires knowing one's body weight and checking it regularly.

Energy density of food

When eating, you should prefer foods with a high energy density. Energy density indicates how many calories are contained in 100g of a particular food. If the energy density is above 250 kcal per 100g, the food has a high energy density.
By understanding the energy density of your favorite foods, you can help manage your weight. For example, a poppy seed roll at breakfast has an energy density of 300 kcal per 100 g, while an apple pie with yeast dough has only 130 kcal per 100 g. Therefore, a single slice of baked good contains either 420 kcal or just 200 kcal. Other examples include sliced cheese, whipped cream, and sausages, which have a high energy density compared to cream cheese, sour cream, and ham, which have a low energy density. By paying attention to energy density, you can eat until you're full without worrying about calorie counts.
When experiencing a loss of appetite, it's best to choose foods with a high energy density and eat them first. You can also increase the calorie content of these meals, for example by adding cream, butter, oil, or maltodextrin.

Protein content of foods

The protein content of food is another important topic. Muscles consist largely of protein, so an adequate protein intake is essential for the respiratory muscles. Current recommendations suggest that healthy individuals should consume approximately 0.8 g of protein per kilogram of body weight daily, which equates to roughly 51 g for someone weighing 64 kg. People with lung diseases, however, should consume more than twice as much protein, namely 1.2–1.9 g of protein per kilogram of body weight.
It's important not to consume your entire protein intake in one meal, as the body can only process a limited amount of protein. A 100g serving of foods like tuna, salmon, or pork tenderloin provides approximately 30g of protein. Legumes such as peas and lentils are also good sources of protein, providing 14g per serving. Eggs, quark, cheese, and peanuts each provide 7-9g of protein per serving.

Tips about food

People with severe lung disease should exert themselves as little as possible when cooking and preparing food. It saves effort and time to prepare larger quantities at once and then freeze them in portions. Those who want to protect their airways while cooking should open the kitchen window or turn on the extractor fan.

Leave the grilling to others and stay away from the grill yourself. Hygiene in the kitchen is even more important than for other people. Accordingly, kitchen sponges should be replaced every 5-7 days, and bacteria and mold should be avoided in general. When cleaning, avoid harsh cleaning agents, as many irritate the lungs when the fumes are inhaled. For this reason, cleaning aids can be a great help. Ready-made meals and frozen foods also reduce the time required and can usually be quickly heated in the microwave before eating. Furthermore, you should always have a selection of different snacks at home, such as dried fruit, granola bars, or cheese cubes. These can be eaten without any effort.

People with lung conditions should eat 5-6 smaller, energy-rich meals rather than just 3 large ones. The advantages are that the meal itself doesn't take as long, and that stomach distension is reduced, leaving more room for lung expansion. It's important to eat as much as possible of foods that you enjoy and find pleasurable. Those experiencing coughs and phlegm should clear their lungs before eating and may want to use a bronchodilator inhaler. While eating, one can sit in a relaxed, upright position, which facilitates breathing. To do this, rest your forearms on the table with your plate between them and keep your legs straight, without crossing them.

Food and liver disease

When liver function is impaired, there are few specific dietary recommendations. In any case, alcohol must be avoided. Furthermore, one should not eat too much fruit or foods sweetened with fructose, as this sugar is metabolized by the liver. Adequate protein intake is also particularly important in this case.

Nutritional therapy and dietary advice

If a normal diet is insufficient to prevent weight loss, nutritional therapy follows a step-by-step approach. Qualified dietitians, including members of the Association of German Dietitians (VDD), assist with individualized treatment.
As a supplementary food, liquid nutritional supplements can be consumed. These contain carbohydrates, proteins, and fats, as well as all essential nutrients, vitamins, and trace elements in the correct amounts. They resemble yogurt drinks and are consumed in addition to regular food. To allow the body to adjust to these supplements, they should be sipped slowly and not consumed before meals. While most liquid nutritional supplements are sweet, there are now also preparations available in neutral or savory flavors. Liquid nutritional supplements are not counted towards daily fluid intake but are considered food. They can be prescribed. If liquid nutritional supplements are unsuccessful, nutrition can be administered via a nasogastric or ileal tube. Only very few patients require intravenous nutrition.

Dangerous interactions: How medications and foods can affect each other

It's common knowledge that medications can interact with each other. Many people are familiar with their doctors' recommendations to avoid taking certain medications together and follow a strict schedule for when to take each medication. However, serious interactions can also occur between medications and food. These interactions can range from reduced or complete loss of effectiveness to life-threatening overdoses. Professor Martin Smollich and Dr. Julia Podlogar, authors of the book "Interactions Between Medications and Food," address this topic, explaining the connections and how to avoid risks.

Water

Taking your medication with a large glass of tap water and while maintaining an upright posture is a good start. This prevents the tablet from sticking in the esophagus, which could delay its effectiveness or, in the worst case, damage the esophageal lining. However, not all water is created equal, and many people take their medication with mineral water. Since both tap and mineral water can be very high in calcium, it's recommended to ensure the calcium content is below 40 milligrams per liter when taking medication. This information is usually found on mineral water bottles. For tap water, you can contact your local water supplier for the calcium levels.

Milk

Milk and dairy products contain calcium and casein. "The calcium in milk strengthens bones" – this isn't entirely wrong, but for people suffering from osteoporosis, for example, this combination can prevent the body from absorbing the active ingredients in their medications. The combination of antibiotics and dairy products, as well as iron supplements and dairy products, is equally problematic.

The tannins in coffee or black tea can negatively affect antidepressants or neuroleptics. The authors explain that there are nitrogen-containing medications in this drug category that, when taken with coffee, can almost completely negate their effectiveness. A warning about nitrogen content is not mandatory for the aforementioned medications, so people with mental health conditions are advised never to take their medication with coffee or black tea. Iron supplements also exhibit reduced absorption when taken with coffee or black tea.

grapefruit

Did you know that grapefruit, often considered so healthy, can lead to serious kidney damage or even death when combined with certain medications? Grapefruit juice contains substances that can cause medications to be present in higher concentrations in the body, potentially leading to unwanted side effects. Therefore, if you are taking, for example, sleeping pills, cholesterol-lowering drugs, blood pressure medication, or antihistamines, you should completely avoid consuming grapefruit. Furthermore, according to Smollich and Podlogar, consuming grapefruit or pomelo can intensify the effects of Viagra and lead to "dangerous cardiac arrhythmias.".

Goji berries

Goji berries are considered a trendy "superfood" and are unfortunately advertised with some dubious health claims. People taking anticoagulant medication should be careful. Goji berry components may intensify the anticoagulant effect and thus lead to dangerous bleeding complications.

high-fiber food

People with Parkinson's disease who are treated with levodopa should not avoid a protein-rich diet, despite the possibility of a protein-related reduction in its effectiveness. It is simply recommended that levodopa never be taken with dairy products and generally 30-60 minutes before a meal. Furthermore, protein-rich meals should ideally be eaten in the evening.

Conclusion

Smollich and Podlogar's book is "heavy reading" for the layperson due to its technical language. However, the topic is highly interesting and reveals many connections that one might not be aware of. This article could only provide a few examples. Hence the standard disclaimer:

These interactions are just a few examples. Therefore, ask your doctor or pharmacist whether your specific medication can interact with food. They can also tell you whether it's sufficient to maintain a time interval, or whether you should completely avoid certain foods for the duration of your treatment.

During my research to make the text understandable for all non-medical professionals, I came across another very interesting connection:

Asthma medication and pepper

This interaction only affects asthma medications containing theophylline, which is present in only 6% of the prescribed asthma medications. Consuming black pepper increases the absorption of theophylline, and the combined effect can lead to nausea, vomiting, and other symptoms.,
It can lead to headaches or even heart rhythm disturbances.
