<p style="font-weight: 400;">Vaccinations have once again become a topic of discussion, especially since Robert Kennedy, a notorious vaccine skeptic, has been at the helm of the US Department of Health and Human Services since last year. &quot;For people with alpha antitrypsin deficiency, however, the benefits far outweigh any potential harm,&quot; stated pulmonologist Camilla Lüttecke-Hecht at the Mainz Patient Day in May, which was hosted by CSL Behring as part of its regular rotation. She recommends alpha-negative vaccinations against influenza, pneumococcus, COVID-19, and RSV. &quot;Because all of these diseases can severely damage the lungs,&quot; the doctor explained.</p>
<p style="font-weight: 400;">In severe cases, the coronavirus led to thrombosis or blood clots in the lungs, which paralyzed gas exchange. RSV can cause veritable cell debris in the airways, and pneumococci fill the alveoli with pus and damage the cilia in the bronchial system. True influenza can even trigger a so-called superinfection. In this case, bacteria also colonize the airways, which are already burdened by the viruses. &quot;The 1918/19 flu pandemic claimed more lives than the entire First World War,&quot; says Lüttecke-Hecht. Estimates suggest up to 100 million victims.</p>
<p style="font-weight: 400;">While no vaccine can prevent disease 100 percent of the time, protection against influenza is between 40 and 60 percent, and protection against pneumococcus between 60 and 75 percent. Vaccinations offer 80 to 90 percent protection against RSV and COVID-19. &quot;However, viral loads are much lower in infected individuals than in unvaccinated people, and the immune system fights the disease more quickly,&quot; says Lüttecke-Hecht.</p>
<p style="font-weight: 400;">Opinions that one can protect oneself against these diseases in this way, or that vaccinations themselves cause the infections, are wrong, she adds. &quot;We can&#039;t vaccinate against a disease,&quot; the physician states. The problem is that many vaccinations are administered in the fall, when there are already many cold viruses circulating. &quot;Some people already carry these viruses when they are vaccinated,&quot; she says. The incubation period, the time between infection and the onset of symptoms, then means that the cold only breaks out after vaccination and is then quickly mistaken for the flu.</p>
<p style="font-weight: 400;">And to avoid the viruses, one would essentially have to wall oneself off for three months, the pulmonologist continued. The viruses or bacteria are transmitted through droplets, for example, during conversation, making avoidance virtually impossible. Furthermore, the immune system needs to be constantly stimulated against viruses and bacteria. &quot;We are still seeing the consequences of wearing masks during the coronavirus pandemic,&quot; she said. The immune system functions by having T cells in the body remember pathogens and sending out B cells, or phagocytes, to fight them. &quot;Actually, we even need the illness to stay healthy,&quot; the doctor explained. But this is where vaccinations come into play.</p>
<p style="font-weight: 400;">The vaccinations provide the T-cells with blueprints or proteins from the viruses, &quot;without transmitting the disease,&quot; as she emphasizes. Because children don&#039;t yet have antibodies, vaccinations are therefore particularly important for them. On the other hand, the immune system weakens with age. &quot;That&#039;s why vaccinations are important not only for children, but also for the elderly and at-risk groups,&quot; she says.</p>
<p style="font-weight: 400;">For pneumococcus and RSV, a single vaccination is sufficient according to current knowledge. Annual vaccinations are necessary against influenza and also against COVID-19 because the viruses are constantly changing.</p>
<p style="font-weight: 400;">The next Mainz Patient Day will take place on November 14th. It is being organized by Grifols and will focus, among other things, on the Alpha-1 Centers. 	<p>Marion Wilkens, Gabi Niethammer and Marion Deter presented the association and the importance of self-help groups and were pleased that both familiar members of the association and new affected individuals participated in the event.</p>
<p>Dr. Natascha Wehnert, senior physician in the Gastroenterology Department, gave a very informative presentation about the liver, a frequently affected organ that should definitely not be overlooked. Among other things, she presented the well-known APRI score, which estimates the extent of liver fibrosis based on the AST (GOT) level and platelet count, as well as the Fib4 score. This score also incorporates the ALT (GPT) level and takes age into account. Both calculation methods can be found on our website under &#039;Alpha-1 and the Liver&#039;. Before the group gathered for a delicious lunch and lively conversation in the hospital&#039;s covered courtyard, Professor Eberhardt had planned something very special: a performance by the choir &#039;Out of Breath&#039; from the Hamburg Conservatory, led by the charming Danish singer Pernille Sieprath. The seven members sang rousing, self-composed songs, both with and without supplemental oxygen, and had the participants on their feet. The use of the kazoo, a small membranophone, was very interesting. Singing into it sets a membrane in motion, thus changing the pitch of one&#039;s own voice. Like singing itself, this also trains the lungs.</p>
<p>What would a patient day be without a pulmonary exercise session? Silke Dammasch led the group in a cheerful and efficient exercise session after the meal, clearing their minds for the subsequent interactive Q&amp;A session: &#039;Who knows about this stuff?&#039;, where the speakers could be peppered with questions on all Alpha-1 topics.</p>
<p>As always, one of the most anticipated presentations concerns the topic of research. Professor Eberhardt presented the ongoing studies and offered hope that some of them are progressing very well.</p>
<p>The very interesting conclusion was given by Prof. Franziska Trudzinski, Senior Consultant of the Department of Pneumology and Respiratory Medicine at the Thorax Clinic Heidelberg. She presented the important EARCO registry and provided insights into the international Alpha-1 community.</p>
<p>Professor Eberhardt and his team organized a very successful patient day for the second time, which, in addition to high-quality specialist information, also allowed ample time for discussion. His wish for an informative, empowering, and inspiring day was perfectly fulfilled for me. Thank you for that.</p>

	<p>On July 19, 2025, CSL Behring hosted the 3rd Alpha1 Antitrypsin Deficiency Patient Day at the Hotel St. Raphael in Kempten, and I was fortunate enough to attend. The event was well-attended, the program promising, the speakers top-notch, and the venue left nothing to be desired.</p>
<p>Right at the beginning, Professor Schumann, Head of Pulmonology at the Allgäu Clinic Network, presented the treatment options for Alpha-1 patients in the Allgäu region. We learned that the clinic in Kempten – an Alpha-1 Center since 2015 – collaborates with Wangen. Mindelheim and Immenstadt are also additional centers where affected individuals can seek care. This information is also helpful when planning a vacation in the Allgäu!</p>
<p>The central theme running through the entire event was the question of how different genders are when it comes to Alpha1-positive disease. In her presentation, Professor Dr. Trudzinski (Thorax Clinic Heidelberg), who joined online from her vacation, explained which gender-specific differences can be identified using an Alpha1 registry. Recommendations for appropriate therapy can only be made with access to large datasets. This is especially true for rare mutations. As we learned, for example, women experience the disease differently, have different inflammatory patterns, different risk factors, different comorbidities, potentially different (occupational) environmental exposures, and more frequent exacerbations than men. Much more research is needed to develop tailored therapies for both men and women.</p>
<p>For a short fitness session, it was Michaela Frisch&#039;s turn with a round of lung exercises. Coordination, flexibility, and strength were required, and the brain was challenged just as much as the muscles.</p>
<p>Dr. Niederwald, a gastroenterologist at the clinic in Kempten, spoke about liver stiffness. She explained the method used to measure liver stiffness at her clinic. Something new and important for me to remember: before the liver stiffness measurement, you should not only be fasting but also rest for at least 15 minutes – so don&#039;t rush to your appointment! We learned a lot about potential liver damage and the differences between liver fibrosis and cirrhosis. The discussion again focused on the differences between men and women. Unfortunately, hormonal changes during menopause can lead to fatty liver disease in women, completely independent of their diet. We women present all agreed that this is extremely unfair!</p>
<p>Professor Dr. Schumann then gave a lecture on lung volume reduction using various types of valves. He explained the anatomy of the lungs as well as the important measurements taken during pulmonary function tests. The Kempten Pulmonary Clinic is a pulmonary emphysema center, so patients there are in excellent hands.</p>
<p>Michaela Frisch also spoke about pulmonary rehabilitation, which should be much more than &quot;just&quot; exercise. Everyday movements should be practiced, relaxation techniques learned, and respiratory muscle strength improved. Fall prevention, endurance, and coordination are important components. The use of respiratory training devices should also be practiced – all tailored to the individual health condition of each participant.</p>
<p>Marion Wilkens concluded by sharing her story – and that&#039;s something all of us affected by Alpha-1 should do. We should all collect our data and spread information about our disease, even if only within our personal circles. Because data can save lives. The better we can inform doctors about the course of our illness, the better we can be treated. And the more data collected in a registry, such as the EARCO study, the more successful the treatment of Alpha-1 can be in the future. Therefore, Ms. Wilkens concluded her presentation at this event by urging patients to participate in studies like the EARCO study and to take part in (reputable) surveys.</p>
<p>A highly informative event concluded with a late lunch together. Many thanks to CSL Behring for making this patient day possible, to the team at the Kempten clinic, and to all those involved. I&#039;m already looking forward to next year&#039;s event.</p>

	<p>„&quot;Same procedure as every year,&quot; one could almost say. For the fourth time in a row, the South German Alpha1 Patient Day took place in Stuttgart. Approximately 130 Alpha1 patients and their families accepted the invitation from Dr. Alexander Rupp (Pulmonology Practice in the city center of Stuttgart). The event was once again held at the Sparkassenakademie near the main train station. The day before, attendees could undergo examinations – essentially lung and liver checks – by the friendly and competent team from RWTH Aachen University and Prof. Pavel Strnad. The examination rooms were well-attended until Saturday afternoon. It&#039;s truly wonderful that so many young medical professionals dedicate their weekend to our event.</p>
<p>Dr. Rupp and Marion Wilkens welcomed the participants early Saturday morning, after which the program proceeded rapidly with a series of informative presentations. Ms. Wilkens briefly outlined the latest news from Alpha1 Germany. Following this, Heike Isensee introduced herself as the group leader and Lothar Sailer as the new deputy leader of the Stuttgart region self-help group.</p>
<p>After Dr. Rupp himself had given an update on asthma, COPD and the various therapy strategies, on comorbidities, vaccination recommendations and much more, Prof. Rembert Koczulla (Schön-Klinik Berchtesgadener Land) was connected online from Salzburg to speak on the topic of advantages and disadvantages of home self-infusion.</p>
<p>Ms. Wilkens then gave a more detailed report on new developments within the association and explained why the data obtained from studies such as the EARCO study are so important for us Alpha1 patients. She encouraged those present to participate in relevant studies to improve the long-term treatment of Alpha1. Dr. Malin Fromme from the University Hospital in Aachen explained the progression from a healthy to a diseased liver and the corresponding diagnostic options, risk factors, and new therapies. Looking ahead to the hopefully not-too-distant future of treatment options is encouraging.</p>
<p>Dr. Tabea Hochstetter from the pulmonology practice in the center of Stuttgart then told us about the various methods one can try to get rid of thick mucus. First, we learned how lung damage leading to mucus buildup can occur.</p>
<p>After a short coffee break, Michaela Frisch led the participants in a pulmonary exercise session, emphasizing movement and coordination. Pulmonary exercise – whether online at home or in person – is one of the most important pillars for maintaining optimal health. To conclude, the participants were asked to share their own &quot;diagnosis journey,&quot; and Dr. Rupp took notes on each story. This resulted in a diverse collection of stories from the &quot;ALPHA1 ON THE GO&quot; series, all centered around this seemingly still too often overlooked disease. Together, we brainstormed ideas on how to raise awareness of Alpha1 among physicians.</p>
<p>It was wonderful to see so many new faces at the event, as well as many familiar ones. The breaks provided ample opportunity for conversation and exchanging the most important information from this training course – namely, insights from those affected, for those affected. The organization by WK-Fortbildungen was reliable. A huge thank you goes to the representatives of the pharmaceutical companies who made this event possible through their generous financial contributions and who were on hand to answer all our questions. And of course, we thank Dr. Rupp, who has been organizing this important patient day in southern Germany for years and is always there for us Alphas.</p>

	<p>Our first patient day was an AlphaCare regional event, supported by the company Grifols, in cooperation with Alpha1 Germany. It took place at the Park-Plaza Hotel in downtown Trier. Several affected individuals attended with their relatives, a total of approximately 15 to 17 people. Dr. Simonetti, a member of Grifols&#039; medical department, provided excellent and helpful information about the disease. Marion Wilkens, the chairwoman of Alpha1 Germany, spoke about the importance of support groups. Afterwards, there was an opportunity to ask questions and engage in conversation.</p>
<p>Around 2010, I started attending information sessions organized by Alpha1 Germany, first in Cologne, then others in Mainz and Heidelberg. I was surprised that hardly anyone from the Trier area was there. This piqued my curiosity, as I figured there must be more people affected by this in the region.</p>
<p>I also contacted the self-help contact point (SEKIS) in Trier. They support the formation of self-help groups, among other things by listing groups on their website and providing meeting rooms.</p>

	<p>People diagnosed with Alpha-1 often feel alone with the whole issue after their diagnosis. They have to painstakingly gather information themselves. While doctors do provide some information, they still have to do a lot of research on their own. Every participant confirmed this experience.</p>
<p>The group is intended for the greater Trier area, the Eifel region, and the Hunsrück region, encompassing a large part of Rhineland-Palatinate, Saarland, and southern North Rhine-Westphalia. The nearest support groups are located in Cologne, Mainz, and Heidelberg. Someone from Hamburg also attended the first meeting; they were there for an examination at the University Hospital Aachen and were able to combine it with our appointment.</p>
<p>The goal is to hold regular meetings and expand the range of topics. For example, I could imagine a future focus on the &quot;effects of Alpha-1 on the liver&quot; or on the topic of oxygen. And of course, we want to attract more affected individuals and interested people from the region to the group.</p>
<p>We are planning another meeting for 2025, likely in the fall (or at the latest, early 2026). You will find the date in the Alpha1 Germany event calendar in due course, or you can send me an email and I will send you the invitation directly.</p>

	<p>Celebrating a birthday: a great opportunity to thank the entire HAEMA team for their work!</p>
<p>In 2024, over 26,000 donors were served at the Hellerdorf distribution center, and on some days more than 200 people come.</p>
<p>Cathrin Platta is the center manager and one of the center&#039;s earliest employees. She enjoys going to work and knows that the tasks she and her team perform daily improve quality of life and save lives. As a patient representative for Alpha1 Germany e.V., I was able to observe her work and would like to share my impressions with you here.</p>
<p>Britta Diebel, responsible for the festive arrangements of the 25th anniversary, welcomed the participants from Haema GmbH, the company Grifols and other guests.</p>
<p>The Governing Mayor Kai Wegner and Nadia Zivkovic, District Mayor of Marzahn-Hellersdorf, received a warm welcome.</p>
<p>In his welcoming address, Kai Wegner remarked on the calm and relaxed atmosphere at the donation center, noting that donors felt comfortable and at ease during their blood and plasma donations. He expressed his gratitude and appreciation to the donors and staff of the Haema Blood and Plasma Donation Center. He was surprised to learn that only three to five percent of citizens donate regularly and promised to focus more on public awareness campaigns to attract more donors and help save lives. He then appealed to all Berliners: &quot;Those who donate blood and plasma regularly save lives.&quot;„</p>
<p>Nadja Zivkovic is campaigning for more blood and plasma donations. She said: “It is important to promote this.“.</p>
<p>Afterwards, a tour of the donation center followed, during which the guests gained insight into the daily work routine. Donors learned about the importance of plasma and its diverse uses through monitors on the walls of the donation room.</p>
<p>An exciting part of the ceremony was the donor recognition at the donor bed. Here, I had the privilege of walking across the &quot;bridge&quot; from donor to patient. All donors received much praise and recognition for their plasma donations, but especially Ronja Moll (34) and Hans-Jürgen Harras (67), who were celebrating their birthdays.</p>
<p>Ronja Moll works at the public health department, and during her lunch break, she saves lives by donating plasma – something she&#039;s been doing for ten years! Her 480th plasma donation was practically a birthday present! She started donating plasma after losing a loved one. She wants to &quot;give something back&quot; and help preserve and save lives.</p>
<p>Hans-Jürgen Harras has been donating plasma since 1997 and has amassed an impressive 927 donations. He is convinced that everyone may need a plasma donation at some point in their lives. For him, it is therefore a matter of course, and as long as he is allowed to donate and thus save lives, he will continue to do so.</p>
<p>Both donors have a wonderful attitude towards plasma donation, are extremely well-informed, and their conscious decision to donate deserves our full respect. As a token of appreciation, they each received a gift, and all the birthday guests thanked them warmly.</p>
<p>I informed them about our very rare disease, Alpha-1 Antitrypsin Deficiency, and that the seriously ill patients would have a shortened lifespan without plasma donors because our life-extending medications would not be available.</p>

	<p>Michael Ruppert informed the birthday guests that donating plasma only takes 45 minutes. The blood is filtered, and the vital proteins contained in the plasma are extracted for the production of medications. Even in its unprocessed form, plasma saves lives by providing care for accident victims.</p>
<p>The anniversary celebration was drawing to a close. Impressions, opinions, and comments were exchanged over coffee and cake.</p>
<p>How wonderful that I was able to join in the celebration; it was a successful anniversary party!</p>
<h2><span style="color: #004267;">We Alphas can also do something about this.</span></h2>
<p>In conversations with acquaintances and friends, serious topics often come up. Talk about the importance of blood and plasma donations. Raise awareness in a friendly way, and who knows, perhaps there are some courageous, healthy people among you who would like to donate blood or plasma.</p>

	<h2><span style="color: #004267;">Resilience in focus: How we as a society and in the healthcare system can become stronger</span></h2>
<p>Under the motto &quot;Resilience – Strengthening Oneself and Others,&quot; the 131st Congress of Internal Medicine of the German Society for Internal Medicine (DGIM) took place in Wiesbaden from May 3 to 6, 2025. With approximately 9,000 participants, Congress President and DGIM Chairman Professor Dr. Jan Galle sent a clear message: Resilience – the ability to cope with stress and grow from challenges – is of central importance, especially for chronically ill people, their families, and medical staff.</p>
<p>In times of skilled labor shortages, structural reforms, and global crises, the question increasingly arises: How can medical care remain humane, sustainable, and of high quality even under difficult conditions – and how can medical staff themselves stay healthy? These challenges affect not only professionals but also patients directly. Resilience is no longer a &quot;nice-to-have&quot; but a fundamental requirement – because only those who maintain a clear head under stress can provide empathetic and competent treatment and avoid errors at the end of a long workday.</p>
<h2><span style="color: #004267;">What is resilience?</span></h2>
<p>Resilience describes mental and physical resilience – and it can be trained. This was also emphasized by Professor Dr. Imad Maatouk, Head of the Department of Psychosomatic Medicine at the University Hospital of Würzburg. Professor Galle, Director of the Department of Nephrology at the Lüdenscheid Hospital, underlined: „The development of resilience is a lifelong process. Our task in internal medicine is to accompany and support patients and medical staff on this path.“</p>
<p>The congress not only brought together experts from clinics, practices, and research, but also addressed societal issues surrounding the topic of resilience. With the participation of the international aid organization Doctors Without Borders, a prominent partner was secured for the first time, one that impressively demonstrates how resilience is practiced under extreme conditions. Dr. Tankred Stöbe, internist and author, emphasized: &quot;Sometimes it helps more to take action than to stand by helplessly – that, too, is resilience.&quot;„</p>
<p>A previously often-overlooked topic also came into focus: climate change. Dr. Martin Herrmann, co-founder and chairman of KLUG e. V. (German Alliance for Climate Change and Health), emphatically drew attention to the health consequences of increasing heat stress. The Robert Koch Institute (RKI) estimates around 3,200 heat-related deaths in the summer of 2023 and approximately 3,000 in 2024 – primarily affecting vulnerable patient groups. Their protection requires adapted structures in hospitals, medical practices, and public life. Relevant expert presentations at the congress provided important insights.</p>

	<h2><span style="color: #004267;">Personal stories: &quot;Focus on what you can influence.&quot;„</span></h2>
<p>A particularly moving contribution was made by Franziska Liebhardt, three-time organ transplant recipient and Paralympic champion, and chairwoman of the association KiO e. V. (Children&#039;s Aid for Organ Transplantation – Athletes for Organ Donation). She spoke openly about setbacks, new beginnings, and the courage to develop new perspectives. Her message: Resilience doesn&#039;t mean invulnerability – but rather the ability to get back up and keep going. Her most compelling statement: &quot;Focus on what you can influence.&quot;„</p>
<p>Dr. Christian Becker, spokesperson for the Young German Society for Internal Medicine (DGIM) and cardiologist at the University Medical Center Göttingen, drew a connection to digitalization. In his view, dealing with new technologies such as artificial intelligence (AI) also requires resilience. The worst solution, he said, is to avoid engaging with digitalization altogether. Ideally, physicians should use the strengths of AI to further develop their own skills in patient care.</p>

	<h2><span style="color: #004267;">Pulmonology and rare diseases in focus</span></h2>
<p>Pulmonology was also represented at the DGIM Congress. A particularly informative lecture on Saturday was given by Dr. Sommerwerck, Prof. Dettmer, and Prof. Vogelmeier. Unfortunately, there was little interest in the topic of bronchiectasis – a cause for concern. If this condition receives so little attention, how difficult must it be to gain recognition for rare diseases like alpha-1 antitrypsin deficiency?</p>
<h2><span style="color: #004267;">Ethics and AI: Challenges for the future</span></h2>
<p>The third day of the congress began with a press conference on a highly topical issue: artificial intelligence in medicine. Professor Hirsch emphasized that AI not only supports diagnostics and documentation but also fundamentally changes the doctor-patient relationship. In the future, physicians will also need to be able to explain the assessments of AI systems in an understandable way – a new dimension of trust in treatment. Alpha1 Germany supports the demand that the use of AI in the sensitive field of medicine must follow clear ethical guidelines.</p>
<h2><span style="color: #004267;">Strong finish with Alpha-1 in focus</span></h2>
<p>A highlight at the close of the congress was the industry symposium hosted by CSL Behring, dedicated to the topic of alpha-1 antitrypsin deficiency – and, to our delight, very well attended. Led by our advisory board members Prof. Dr. Trudzinski (Heidelberg) and Prof. Dr. Koczulla (Schönau), as well as Dr. Rupp (Stuttgart), also an advisory board member of Alpha1 Germany, who contributed the practical perspective of a practicing pulmonologist, the symposium focused on raising awareness of alpha-1. A successful conclusion to our congress visit. Whether physician, patient, or family member – resilience is a topic that affects us all. The 2025 DGIM Congress impressively demonstrated how we, in our daily medical practice and as a society, can learn to cope with crises, strengthen one another, and find new paths forward. Alpha1 Germany was right in the thick of it – and we are taking away many important insights.</p>

	<p>AATM diagnosis in 2024. For the first time at a members&#039; meeting! The spa town of Bad Wildungen presents itself invitingly in the spring sunshine; the Hotel Maritim, situated in the spa gardens, still exudes the scent of bygone golden ages of glorious spa seasons. Blossoming trees and daffodils, a babbling brook.</p>
<p>At the start at 5 p.m., the foyer is quite full, with a palpable sense of joy at seeing each other again. As a newcomer, you might find yourself standing around for a bit, but it&#039;s not unpleasant; there&#039;s always a cup of coffee to hold onto. The main hall is already prepared, with the seats for the board members, the lectern, the sound system, and the technical equipment set up on the stage.</p>
<p>The attendance list shows 174 participants; the atmosphere is friendly and attentive. Initial contact is made with those sitting next to each other.</p>
<p>The formalities of the members&#039; meeting get underway, warmly introduced by the chairwoman, Marion Wilkens. Gabi Niethammer then skillfully takes over the moderation; after all, German association law must be observed alongside the important business. Participating in the proceedings is effortless; everyone knows it&#039;s necessary. The meeting moves smoothly through the agenda.</p>
<p>The annual report was available in advance, but it&#039;s still great to see it presented live and in such an engaging way. As a newcomer, you can really feel the dedication. Such a wide range of activities, including those at the international level! It&#039;s somehow impressive that the association already has over 1,000 members. There&#039;s so much they want to do, so much they want to achieve for everyone currently affected and for those in the future! They also spoke openly about the finances, including how they covered the hotel costs for this weekend. As someone hearing much of this for the first time, I&#039;m very impressed. I&#039;m grateful to the volunteers who are willing to take on the board positions again for another term and were elected. In the extensive network of associations I&#039;m usually involved in, this is truly remarkable. Respect! The division of tasks, the auditing – everything was handled properly. It wasn&#039;t boring at all!</p>
<h2><span style="color: #004267;">Dinner is calling.</span></h2>
<p>The crowd trots through long hotel corridors in loose groups towards the dining room. I find it amusing that this herd instinct is framed by animal photos on the walls: lots of sheep. The very diverse and plentiful dinner offers new opportunities to connect, facilitated by a glass of wine. But the conversation topics aren&#039;t limited to the usual suspects; they also arise pleasantly from small talk and the garden&#039;s sensory experiences.</p>
<h2><span style="color: #004267;">Day 2, Information Day</span></h2>
<p>An early morning stroll in the spa gardens is a good idea. I&#039;m scheduled for a liver examination, and having my blood drawn in the beautifully renovated Art Nouveau architecture is quite pleasant, I discover. Perhaps the spa aristocracy felt this way.</p>
<h2><span style="color: #004267;">A full day follows</span></h2>
<p>On a variety of topics, there&#039;s a fearless encounter between science and the general public; even complex diagrams and charts are presented to the audience, which I find very appealing. Questions are asked openly and answered most kindly, sometimes with the suggestion of later one-on-one discussions. The atmosphere is encouraging, and the accompanying organization is clever and respectful. Hats off! I&#039;m sure there will be further reports on individual topics on the website and in the association&#039;s magazine. Children are present, and childcare is provided and clearly well-used. Information booths offer a wealth of materials, making your rolling suitcase quite heavy. And I&#039;ve had excellent results asking questions about the topic to those enjoying coffee nearby. The sense of community is palpable.</p>
<h2><span style="color: #004267;">Farewell on the little train</span></h2>
<p>And this too: Via Wega and Ungedanken, Zennern and Mandern (!) you get back to the main route. Fantastic. I drive home grateful, with the assurance that this association is a virtual place where knowledge and experience are freely shared, where I don&#039;t have to be afraid of asking silly questions. I can read about, ask about, and express my concerns here. This is what support looks like, this is what self-help looks like when it works. I&#039;m looking forward to the next regional meeting.</p>

<li style="font-weight: 400;">Alpha1 Patient Day in Kempten in the Allgäu</li>
</ol>
<p style="font-weight: 400;">On July 19, 2025, CSL Behring hosted the 3rd Alpha1 Antitrypsin Deficiency Patient Day at the Hotel St. Raphael in Kempten, and I was fortunate enough to attend. The event was well-attended, the program promising, the speakers top-notch, and the venue left nothing to be desired.</p>
<p style="font-weight: 400;">Right at the beginning, Professor Schumann, Head of Pulmonology at the Allgäu Clinic Network, presented the treatment options for Alpha1 patients in the Allgäu region. We learned that the clinic in Kempten – an Alpha1 Center since 2015 – collaborates with Wangen. Mindelheim and Immenstadt are also additional centers where affected individuals can seek help. This information is also helpful when planning a vacation in the Allgäu!</p>
<p style="font-weight: 400;">The common thread running through the entire event was the question of how much of a difference it makes whether one is a woman or a man suffering from Alpha1.</p>
<p style="font-weight: 400;">In the following presentation by Professor Trudzinski (Heidelberg Thoracic Clinic), who joined online from her vacation, the question of which gender-specific differences can be identified using an Alpha1 registry was addressed. Recommendations for appropriate therapy can only be made when large datasets are available. This is particularly true for rare mutations. As we learned, for example, women experience illness differently, have different inflammatory patterns, different risk factors, different comorbidities, possibly different (occupational) environmental stressors, and more frequent exacerbations than men. Much research is still needed to develop therapies that are &quot;appropriate&quot; for both men and women.</p>
<p style="font-weight: 400;">For a short fitness session, it was Michaela Frisch&#039;s turn with a round of lung exercises. Coordination, flexibility, and strength were required, and the brain was challenged just as much as the muscles.</p>
<p style="font-weight: 400;">Dr. Niederwald, a gastroenterologist at the clinic in Kempten, spoke about liver stiffness. She explained the method used to measure liver stiffness at her clinic. Something new and important for me to remember: before the liver stiffness measurement, you should not only be fasting but also rest for at least 15 minutes – so don&#039;t rush to your appointment! We learned a lot about potential liver damage and the differences between liver fibrosis and cirrhosis. The discussion again focused on the differences between men and women. Unfortunately, hormonal changes during menopause can lead to fatty liver disease in women, completely independent of their diet. We women present all agreed that this is extremely unfair!</p>
<p style="font-weight: 400;">Professor Schumann then gave a lecture on lung volume reduction using various types of valves. He explained the anatomy of the lungs as well as the important measurements taken during pulmonary function tests. The Kempten Pulmonology Clinic is a pulmonary emphysema center, so patients there are in excellent hands.</p>
<p style="font-weight: 400;">Michaela Frisch also spoke about pulmonary rehabilitation, which should be much more than &quot;just&quot; exercise. Everyday movements should be practiced, relaxation techniques learned, and respiratory muscle strength improved. Fall prevention, endurance, and coordination are important components. The use of respiratory training devices should also be practiced – all tailored to the individual health condition of each participant.</p>
<p style="font-weight: 400;">Marion Wilkens concluded by sharing her story – and that&#039;s something all of us affected by this condition should do. We should all collect our data and share information about our illness, even if only within our personal circles. Because data can save lives. The better we can inform doctors about the course of our illness, the better we can be treated. And the more data collected in a registry, such as the EARCO study, the more successful the treatment of Alpha-1 can be in the future. Therefore, Ms. Wilkens also concluded her remarks at this event by urging patients to participate in studies like the EARCO study and to take part in (reputable) surveys.</p>

										<content:encoded><![CDATA[<p style="font-weight: 400;"><strong>Report from the 2nd MHH Patient Seminar „Alpha-1 Antitrypsin Deficiency“ at Hannover Medical School on August 24, 2024</strong></p>
<p style="font-weight: 400;"><strong> </strong></p>
<p style="font-weight: 400;">At the beginning, Professor Dr. Sabina Janciauskiene from the Department of Pneumology and Infectious Diseases at Hannover Medical School (MHH), as scientific director of the event, and Marion Wilkens, board member of Alpha1-Deutschland e.V., welcomed the large number of guests. They also immediately expressed their gratitude to those who supported and made the event possible: Dr. Annegret Zurawski, clinic manager at MHH, for organizing and conducting the event; Inga Kwapniewska from the German Center for Lung Research (DZL) at MHH for her organizational support; the sponsors Grifols, CSL Behring, and Takeda for their financial support; and Linde for providing the oxygen.</p>
<p style="font-weight: 400;">The first presentation was given by Dr. Isabell Pink on the topic of &quot;Bronchiectasis in AATD.&quot; Bronchiectasis refers to the widening and dilation of the bronchi, the airways between the trachea and the alveoli. Since AATD itself is a rare disease, its contribution to the overall incidence of bronchiectasis plays only a minor role, as can be seen from analyses of the corresponding European Bronchiectasis Registry EMBARC. However, studies based on the European Alpha-1 Registry EARCO have shown that a large proportion of individuals with the PI*ZZ variant of AATD exhibit bronchiectasis, usually in addition to pulmonary emphysema (27% of all Alphas have both).</p>
<p style="font-weight: 400;">Treatment for bronchiectasis initially focuses on the underlying condition, such as asthma or COPD, primarily with inhaled medications. Smoking cessation is also mandatory, and respiratory physiotherapy and measures to loosen secretions (e.g., using nebulizers) are helpful. However, these measures only alleviate symptoms and may prevent inflammation; the tissue changes themselves are irreversible.</p>
<p style="font-weight: 400;">The aforementioned deformities of the bronchi lead to the accumulation of mucus, which provides an ideal breeding ground for all kinds of pathogens. This often results in chronic bronchiectasis, a condition for which two of the following three criteria must be met for diagnosis:</p>
<ul>
<li>Cough on most days of the week</li>
<li>Mucus production on most days of the week</li>
<li>experienced exacerbations (su)</li>
</ul>
<p style="font-weight: 400;">These chronic symptoms can sometimes develop into acute, crisis-like inflammatory processes, known as exacerbations. An exacerbation is suspected if at least three of the six key symptoms occur or worsen for at least 48 hours.</p>
<ul>
<li>Cough</li>
<li>Ejection</li>
<li>Discoloration in the sputum</li>
<li>shortness of breath</li>
<li>fatigue</li>
<li>Coughing up blood</li>
</ul>
<p style="font-weight: 400;">Fever, inflammation markers, and other clinical abnormalities can also support the diagnosis.</p>
<p style="font-weight: 400;">The therapy to be applied depends on the severity of the symptoms; it ranges from physiotherapeutic drainage of secretions to oral antibiotics and intravenous high-dose antibiotics as part of inpatient treatment.</p>
<p style="font-weight: 400;">Approaches to reducing the frequency of exacerbations depend on any identified triggers:</p>
<ul>
<li>If the self-cleaning function of the airways is impaired, it must be strengthened, e.g. through appropriate medication and targeted respiratory physiotherapy.</li>
<li>In cases of bacterial infections, the aim is to permanently eliminate the relevant bacterial strains (e.g., Pseudomonas aeruginosa) through targeted antibiotic treatment; incidentally, patients with bronchiectasis and an acute Pseudomonas aeruginosa infection can currently (autumn 2024) apply to participate in a drug trial at [website address missing]. <a href="https://www.lungeninformationsdienst.de/klinische-studien/aktuelle-klinische-studien/non-cf-bronchiektasen/eradicate">https://www.lungeninformationsdienst.de/klinische-studien/aktuelle-klinische-studien/non-cf-bronchiektasen/eradicate</a></li>
<li>In cases of respiratory tract inflammation, therapy is based on the functional cause of the inflammation.</li>
</ul>
<p style="font-weight: 400;">If the underlying cause is neutrophilic, a drug is now nearing the end of clinical trials: Brensocatib from Insmed. Phase 3, the comparison of efficacy with a placebo, has been successfully completed and largely evaluated: treatment with Brensocatib resulted in significantly fewer exacerbations during the observation period than in the placebo group, and the time to the first exacerbation was significantly prolonged; no serious adverse events were observed. The approval process has been initiated, so availability for patients is expected from 2026 onwards. It is anticipated that AATD patients with the corresponding clinical picture will also benefit from treatment with Brensocatib.</p>
<p style="font-weight: 400;">Following this positive outlook, in her subsequent presentation, &quot;Living with AATM,&quot; Marion Wilkens, chairwoman of Alpha1-Germany, shared her life story with the audience: as a baby, she suffered from severe jaundice and failure to thrive, which, with today&#039;s knowledge, were already early signs of her illness. At 20, she was diagnosed with asthma, and it took another 20 years until she received an AATM diagnosis. Since then, her condition has been managed with regular lung and liver examinations, plenty of exercise, respiratory physiotherapy, and an adapted lifestyle. Among other things, she has been helped in coping with the illness by the wealth of information she has found, some of which she found readily available, and some of which she has gathered herself through her work with the association and presented in her talk. She emphasized once again the value of a large, well-informed, and mutually supportive community like the Alpha1 Germany association as a building block for successfully managing AATM, and then presented its structure and work in all its facets: regular events, self-help groups, information channels online and in print, free pulmonary exercise and memory training, and networking with organizations of doctors and patients (including internationally). All of this serves the goal of enabling those affected to live better lives with AATM.</p>
<p style="font-weight: 400;">
<p style="font-weight: 400;">The third presentation was given by Dr. Hinze on the topic of &quot;Infections and Vaccination in AATD.&quot; He based his presentation on the recommendations of the nationwide Standing Committee on Vaccination (STIKO). While the primary goal is to avoid the infections in question and their often very unpleasant side effects, the aspect of preventing additional cardiovascular strain in patients with chronic pre-existing conditions and advanced age also plays a crucial role.</p>
<p style="font-weight: 400;">The following vaccinations are recommended:</p>
<ul>
<li><strong>Pneumococci</strong> to prevent pneumonia (the vaccine is selected by the vaccinating doctor based on the individual&#039;s vaccination status)</li>
<li><strong>Respiratory syncytial virus (RSV)</strong> to prevent exacerbations; at the time of the presentation, cost coverage still had to be arranged individually with the respective payer, but in the long term, it is expected that health insurance companies will cover the costs for high-risk patients in late 2024.</li>
<li><strong>SARS-CoV2 (Covid-19)</strong>The vaccination should be boosted annually; after infection, the booster can be omitted for one year.</li>
<li><strong>Influenza (flu)</strong> as a high-dose vaccination for people over 60 years of age, also with regard to preventing the exacerbation of cardiovascular crises (heart attacks!).</li>
<li><strong>Herpes zoster (shingles)</strong> as an inactivated vaccine; even if there is a noticeable reaction to the first vaccination, the second vaccination should definitely be carried out after 2 to 6 months.</li>
</ul>
<p style="font-weight: 400;">
<p style="font-weight: 400;">Dr. Mark Greer then gave a presentation on the topic of &quot;Lung Transplantation in AATD.&quot; This procedure should be considered when lung function is largely lost and no further treatment options are available. However, transplantation is subject to a number of other prerequisites, including:</p>
<ul>
<li>Age</li>
<li>body weight</li>
<li>Compliance with the smoking ban</li>
<li>no cancers</li>
<li>no diseases of other vital organs</li>
<li>psychological stability</li>
<li>Ability/willingness to undergo rehabilitation</li>
</ul>
<p style="font-weight: 400;">In addition to these objective criteria, assessments by the treating physicians also play a role, e.g.:</p>
<ul>
<li>a mortality risk above 50% within the next 2 years of life</li>
<li>A survival probability of over 80% for the next 5 years of life after transplantation</li>
</ul>
<p style="font-weight: 400;">If all these conditions are met, a patient can be considered for a lung transplant. This will then take place as soon as a suitable donor organ is available that is not needed for a patient with an even greater need for a transplant.</p>
<p style="font-weight: 400;">In the vast majority of cases, the patients&#039; quality of life is significantly improved by the transplant, even though life afterwards differs significantly from life before: for example, medication must be taken regularly for life to prevent rejection of the new lung, and close monitoring by the transplant center remains necessary for years to come.</p>
<p style="font-weight: 400;">The success rate of lung transplants is constantly improving: half of all lung transplant recipients now survive the operation for more than 6.5 years.</p>
<p style="font-weight: 400;">
<p style="font-weight: 400;">Following this presentation, Prof. Dr. Sabina Janciauskiene addressed the question &quot;Substitution – yes or no?&quot;, illustrating her points with reference to real-life case studies. She began by describing the mechanism of action of substitution therapy, which aims to compensate for the unbalanced activity of neutrophil elastase that leads to the breakdown of lung tissue. Alternatives to the common intravenous administration of AAT are currently under development: inhalation, transdermal administration, and the use of recombinant (i.e., biochemically produced) AATs. The latter is currently being tested in a clinical trial: this variant is metabolized more slowly by the body, potentially allowing for significantly longer treatment intervals, but it is also associated with a number of side effects.</p>
<p style="font-weight: 400;">With conventional replacement therapy using a weight-dependent dose, the treatment success for individual patients is sometimes difficult to predict, as examples from clinical practice demonstrate. Determining the dose based on the existing AAT level and other clinical parameters may offer advantages. Patients with poorer lung function than currently anticipated could also likely benefit from replacement therapy. However, no improvements have been observed in patients with bronchiectasis or asthma. The continuation of therapy after lung transplantation, on the other hand, remains controversial.</p>
<p style="font-weight: 400;">It is often overlooked that AATD can trigger a host of skin problems, often indirectly via AATD-related intestinal issues. These include, for example, acne, eczema, psoriasis, rosacea, urticaria, and dermatitis herpetiformis.</p>
<p style="font-weight: 400;">
<p style="font-weight: 400;">The final presentation of the day was given by Dr. Katja Deterding on the topic of &quot;AATD from a Hepatologist&#039;s Perspective – New Treatment Options.&quot; She began by explaining that AATD involves an imbalance between AAT and neutrophilic elastase (NE), which can lead to the enzymatic destruction of lung tissue, resulting in pulmonary emphysema. This imbalance does not arise from insufficient AAT production in the liver, but rather from AAT being misfolded, causing it to polymerize into clumps or strands so large that they cannot leave the liver cells that produce them. This can lead to liver fibrosis, cirrhosis, or even liver cancer. While this process is particularly pronounced in individuals with the homozygous PI*ZZ variant, those with the heterozygous PI*MZ variant have more AAT entering the bloodstream, thus largely protecting the lungs, at least in non-smokers. However, in these patients, the risk of liver complications is significantly increased if chronic liver diseases are also present.</p>
<p style="font-weight: 400;">To detect and classify liver fibrosis, a liver tissue biopsy is the most reliable method, but also the one with the highest risk of adverse side effects. In contrast, measuring liver elasticity using FibroScan is free of side effects and therefore very suitable for monitoring disease progression. For monitoring alphas with the ZZ classification, the following are recommended:</p>
<ul>
<li>Annual liver function tests, possibly more frequent if taking potentially liver-damaging medications.</li>
<li>Fibroscan every 3 years if the results are normal, otherwise more frequently (alternatively, the APRI value can be calculated from measured liver values; see also the link to an APRI calculator on the Alpha1 Germany website at [link to APRI calculator]). <a href="https://alpha1-deutschland.org/en/alpha-1-und-die-leber/">https://alpha1-deutschland.org/alpha-1-und-die-leber</a>)</li>
<li>Liver function test in advanced disease</li>
<li>Cancer monitoring in advanced cirrhosis</li>
<li>Even though liver and lung problems only very rarely occur together in ZZ patients, the lungs should be checked regularly even if abnormal liver findings are present.</li>
</ul>
<p style="font-weight: 400;">
<p style="font-weight: 400;">Novel therapeutic approaches for AATM look like this:</p>
<ul>
<li>The intake of chemical agents is intended to correct the misfolding of AAT so that it passes from the liver into the blood in sufficient quantities to avoid burdening the liver with polymers and to adequately protect the lungs.</li>
<li>By intervening in the genetic material (DNA) or in the copies of individual DNA segments necessary for the formation of proteins (i.e., based on RNA), the liver is to be made to produce &quot;normal&quot; AAT.</li>
<li>Similarly, RNA-based interventions can downregulate the production of AAT, thereby allowing a liver burdened by AAT clumping to recover; a clinical trial of this approach has been underway in Germany for some time and is currently being expanded significantly, for which further study participants are being sought; interested parties can find further information, including the inclusion and exclusion criteria for participation, online at [website address]. <a href="https://global.theredwoodliverstudy.com/de-ch">https://global.theredwoodliverstudy.com/de-ch</a></li>

<p style="font-weight: 400;">This presentation concluded the second MHH patient seminar on &quot;Alpha-1 Antitrypsin Deficiency.&quot; The large number of participants, the numerous questions and lively discussions with the speakers, and the intensive exchange of opinions among the attendees give them hope for further events in this seminar series.</p>
