Aspects of rehabilitation, a journal article from 2017 by Michaela Frisch

When should one consider rehabilitation, and what conditions should be met?

If outpatient treatment options have been exhausted and persistent symptoms of illness remain, or if the ability to function in daily life is further reduced, thus jeopardizing independent living, then rehabilitation is required. This includes the need for an interdisciplinary therapeutic approach (e.g., nutrition, psychological support, exercise therapy, patient education, etc.) to improve and stabilize participation in daily life, provided that the therapies cannot be carried out to the necessary extent on an outpatient basis.

The need for rehabilitation can also be justified to avoid a more costly hospital stay due to a worsening of the condition or an impending need for long-term care that rehabilitation can prevent. It can also exist after a hospital stay, in which case it is referred to as AHB (follow-up treatment, with costs covered by the German Federal or State Pension Insurance) or AR (follow-up rehabilitation, with costs covered by the health insurance fund). Applications are submitted through the treating physician and the social services department.

Rehabilitation offers an opportunity to stabilize the course of an illness and prevent its deterioration, but it also requires the patient's willingness, motivation, and resilience. The patient's mental and physical condition is therefore essential for rehabilitation. Non-invasive ventilation (NIV) or long-term oxygen therapy (LTOT) are not exclusion criteria, but when choosing a clinic, one should pay attention to its specialization.

The treating physician should describe the rehabilitation prognosis as positive. Even though rehabilitation requires a certain degree of personal responsibility and willingness on the part of the patient, the rehabilitation goals should be realistic and defined in relation to the patient's individual circumstances and the improvement in quality of life resulting from the rehabilitation measures.

Special requirements and quality criteria of the payers for the clinics

When searching for a suitable rehabilitation clinic within the framework of the right to choose, in addition to one's own experiences, it is also advisable to look at the so-called special requirements and quality characteristics of the cost bearers for the clinics:

Specialization, e.g. also in rare lung diseases

Pulmonologists (lung specialists) available in-house all day (and not just for a few hours once a week)

Other areas of specialization within the qualification: Internal medicine, allergology, sleep medicine

Specialization also includes the provision, prescription, training, and monitoring of long-term oxygen therapy and non-invasive ventilation.

Additional qualification in diabetology may be required due to comorbidity.

Possibly specialized training in the field of obesity and allergology for the interdisciplinary team

Possibly cooperation with the regional dialysis center, the regional radiology practice, as well as dermatologists and ophthalmologists or specialists in neurology and cardiology.

Cooperation with a speech therapy practice

Daily patient admission (including nursing and medical admissions)

Doctors and qualified nurses are on duty at the clinic 24 hours a day, 365 days a year.

Accommodation for accompanying persons is possible

Individualized application plan

The execution of tests and the evaluation of the outcome parameters will be published.

Good preparation is key.

As is often said, preparation is half the battle. You will need:

All relevant documents

The most recent examination results from the last 3-6 months from doctors or examinations in clinics.

A reasoned statement from the treating pulmonologist regarding the need for rehabilitation, the rehabilitation prognosis, and the rehabilitation goals.

Personal statement regarding the restriction of participation, the reduction of social and general mobility, reduced workload and performance in everyday life, etc.

Personal statements regarding one's own rehabilitation goals: "What do I want to achieve in rehabilitation?", "What did I take away from the last rehabilitation and integrate or change in my everyday life?"„

If available: a positive discharge report from the last rehabilitation measure regarding the improvement in physical resilience, adherence to therapy, stabilization of the disease course, increase in measurable parameters (walking test distance, lung function values, etc.), implemented behavioral recommendations.

What are the steps to take if a planned rehabilitation is rejected?

Should the rehabilitation application be rejected despite thorough preparation, it is very important to note the date of receipt on the letter in order to keep track of and comply with the applicable legal deadlines for objections or legal action.

If a preferred clinic has been named in the application, it is best to involve them as a partner and request a corresponding statement and support.

Please do not use pre-formulated letters, but instead provide individually justified and detailed information in your letter of objection and send it to the cost bearer by registered mail, taking into account the deadlines.

What is pulmonary rehabilitation?

Pulmonary rehabilitation is a comprehensive intervention that, based on the identification of each individual's specific problems, designs a specialized therapy program. In addition to physical training and education, this program primarily aims to change behavior. The goal is to improve the mental and physical well-being of patients with chronic respiratory diseases and to promote a long-term shift towards a more health-conscious lifestyle. This is also reflected in the corresponding evidence base, which is rated at level A.

Increased physical performance

Improvement of strength and endurance of the leg muscles (evidence level B for arm muscles)

Reduction of hospital stays and sick days or hospital admissions

Improving quality of life

Improvement of psychological components such as anxiety and depression

Reduction of shortness of breath

The need for rehabilitation can also be justified to avoid a more costly hospital stay due to a worsening of the condition or an impending need for long-term care that rehabilitation can prevent. It can also arise after a hospital stay, in which case it is referred to as AHB (follow-up treatment, with costs covered by the German Federal or State Pension Insurance) or AR (follow-up rehabilitation, with costs covered by the health insurance fund).

What would pulmonary rehabilitation be without guidance and transition to aftercare?

In addition to all the self-help instructions and home programs, there are other important recommendations:

Participation in Pulmonary exercise

Advice and information

Prescription is possible at least for those insured by the German Pension Insurance (DRV) federal or state governments in the rehabilitation clinic.

information

Referral to self-help groups

Information material, Flyer and brochures

Because the overall package is what makes pulmonary rehabilitation sustainably effective. The patient, however, must be willing and actively participate in order to gain as much as possible for themselves and implement and integrate it into their daily life.

PROMISE goes into overtime!

Since October 2022, patients with Alpha-1 Antitrypsin Deficiency have had the opportunity to participate in our personalized training and respiratory physiotherapy program PROMISE, and we are very pleased that the program is being extended with another 25 places!

What is changing?

The program will now nationwide Offered: To also be able to provide care to interested patients who do not live in our catchment area, we have switched PROMISE to a purely digital program. We will, of course, provide support with any technical questions.

What remains the same?

Our goal: Most people know that physical exercise and knowledge of how to manage alpha-1 antitrypsin deficiency and COPD significantly contribute to a good quality of life. But how can you effectively integrate targeted training into your daily routine? What simple tricks can help you control and improve shortness of breath during exertion (and the fear of it)? And which exercises are best suited for you personally?

To optimize disease management outside of drug therapy, we have launched the PROMISE program, which is specifically designed for Alpha-1 Antitrypsin Deficiency patients.

The content:

PROMISE is offered by Pneumo Factory (located in Schönau am Königssee) with financial support from CSL Behring. The program aims to empower you to become a competent manager of your condition by providing non-pharmacological therapy components tailored to your individual needs. A training plan for endurance and strength training is created, which can be performed in your own home without the need for any equipment. Furthermore, essential respiratory physiotherapy techniques, such as pursed-lip breathing, are reviewed and taught, and practical information is provided on topics such as mental health, long-term oxygen therapy, and nutrition.

How long does the program last?

The program initially lasts 8 weeks and includes 6 digital appointments via video conference (using a data protection-compliant platform) with a therapist from Pneumo Factory (physiotherapists, sports scientists). All appointments are spaced approximately 2 weeks apart (by individual arrangement), allowing participants time to independently try out the new training plan in the interim. The long-term goal is for the training to be integrated into daily life and carried out independently and regularly! Furthermore, a digital follow-up session takes place 8 weeks after the last regular appointment (Figure 1).

How can I tell if the program was effective?

At the first appointment, a comprehensive assessment is conducted according to scientific standards, allowing for an evaluation of physical performance, quality of life, and symptom intensity. These assessments are repeated at the fifth appointment to track any changes. To examine the long-term benefits of the program, a final, sixth appointment takes place eight weeks after the fifth, repeating all assessments. Naturally, all participants will receive feedback on their personal progress upon request.

Who is behind PROMISE?

The content was developed by Pneumo Factory (Dr. Tessa Schneeberger [MSc. Sports Physiotherapist, Respiratory Physiotherapist] and Dr. Inga Jarosch [Dipl. Sports Scientist], both research associates and speakers for the Respiratory Physiotherapy Working Group, the Respiratory Therapist Training Program, and the Heart LAG Training Program) based on current scientific findings (www.pneumo-factory.de). Thanks to the generous support of CSL Behring, PROMISE can be offered free of charge to patients.

Who can participate?

Any patient with alpha-1 antitrypsin deficiency who suffers from COPD and feels limited in their daily life by the disease can participate in PROMISE free of charge. To rule out any medical contraindications for training before participating in PROMISE, training approval from your general practitioner or pulmonologist is required (the form is available for download at www.pneumo-factory.de or upon request at info@pneumo-factory.de). To date, 46 participants have successfully completed PROMISE.

What do PROMISE participants say?

„…The therapist was a stroke of luck, because he was not only very personable, but also highly competent and goal-oriented. I quickly realized this, as he identified my shortcomings, weaknesses, and exercise needs in a short time. (…) I was praised, because the targets set at our first meeting were exceeded. I had already noticed this myself, as my shortness of breath when climbing stairs or walking uphill had decreased, my strength had increased, and I felt fitter overall…“

…“Whether my empyema (…) has improved, I cannot say, but I definitely know that I can breathe much better, that I no longer have panic attacks of shortness of breath, and especially… I am no longer afraid of climbing stairs! The correct technique, pursed-lip breathing, the right pace, the correct inhalation and exhalation, etc., I had previously paid too little attention to all of that…“

„…The therapist ensured that I could easily integrate my training sessions into my daily routine and also addressed any health setbacks. All in all, my physical performance has noticeably improved, and I'm coping much better with everyday life. Climbing stairs, in particular, is now easier than at the beginning. The program also motivated me to stay committed and continue working on my fitness, nutrition, and self-care. I'm very grateful that I was able to participate in the program and would gladly recommend it…„

Are you also interested in participating in PROMISE or do you have any questions about it? Then please contact us by email: info@pneumo-factory.de. We are happy to answer all your questions about the program!

After undergoing rehabilitation at the Berchtesgadener Land Clinic in Schönau am Königssee in 2018 and 2020, I wanted to breathe in some sea air again this year, which is beneficial for most of us with lung conditions. After browsing the very positive reviews on the website "Klinikbewertungen.de", I chose the Baltic Sea Clinic.

I submitted my application for rehabilitation back in January 2022, after a check-up at the Ruhrland Clinic, where it was recommended. Unfortunately, my family doctor made a mistake on the application. She checked "no restrictions" everywhere, which led to the rehabilitation being rejected. I didn't notice the error either, because she only filled out the application after I asked her several times and handed it to me in a rush the day before my vacation. To avoid wasting any more time, I quickly scanned the application and sent it by email. I then filed an appeal against the rejection, which was ultimately approved. In addition, the waiting times due to the pandemic were so long that I was finally able to start my rehabilitation in May or June 2023.

Up until now, I've always taken public transport to my rehabilitation clinic. This time I opted for the car, as traveling with an oxygen concentrator and changing trains would be quite complicated. It's just over 600 km from my hometown of Duisburg to Prerow, a distance I was able to cover without any traffic jams on both the outward and return journeys.
Arriving on the Darß peninsula, I was overwhelmed by the magnificent landscape and the elaborately restored houses in the North German style. At the hospital, I received a friendly welcome at reception and was able to park my car in a gated parking lot for €2 per day. The hospital is located right on the beach, which is a maximum 5-minute walk away.

After settling into my lovely room with a balcony, I went straight to dinner. Two new companions were waiting for me there, with whom I later went on several bike rides. Dinner consisted of the usual bread with various cold cuts.

The next day I went for my initial examination, which included lung function tests and a gait test. The ward physician asked me about my preferences regarding treatments, and from my perspective, a well-balanced plan was created, including water aerobics in the very nice, large in-house swimming pool, moderate Nordic walking, and strength training. To put it bluntly: the strength training was significantly lacking. While the equipment was modern, it was rather basic. As a result, the time available for training was very limited. This is quite different from what I'm used to at the Berchtesgadener Land Clinic. There, patients are encouraged to use the equipment every day, as muscle building is very important for people with lung conditions.

Between treatments, patients could relax in a lovely, bright atrium near reception, which also served as a cafeteria and event space. However, I found the cafeteria prices outrageous. A coffee cost €3 and a bottle of beer €4.90. Those without mobility issues could easily find nearby restaurants. But there were several seriously ill patients who couldn't walk and were forced to pay these exorbitant prices. It's especially difficult for those who are already struggling financially.

The next day at lunchtime, I felt like I wasn't in a rehab clinic, but in a well-run restaurant. Almost every day there was a choice between fish and meat. A vegetarian option was also available. In addition, there was soup, a salad bar, and dessert. I only ate out once during the four weeks, the food there was so delicious. It was nothing like the dreadful food at the Berchtesgadener Land Clinic, which cost me a lot of money and generated considerable revenue for the nearby restaurants.
When I had a break between treatments, I usually went to the beach and walked barefoot through the water. I was also incredibly lucky with the weather; it was only overcast on two days during the four weeks. Otherwise, we always had bright blue skies and temperatures around 22 degrees Celsius. Incidentally, I had checked beforehand whether I would be supplied with liquid oxygen at night and whether I could recharge my portable oxygen concentrator myself. In Heiligendamm (on the Baltic Sea), where I was for rehabilitation in 2017, they provided me with a concentrator in my room, which was so loud I couldn't sleep a wink – after I asked, they then provided me with a stroller at night. You weren't allowed to recharge your own portable devices either; the nurses did that. So, several times a day, I sat outside the nurses' station, practically begging for help, and often received my Helios H300 back frozen solid, which meant the device wouldn't release any oxygen for about an hour. This forced me to miss some treatments. An unacceptable situation that didn't improve even after I complained to the head physician. But that's just a side note. The physiotherapists were all absolutely fantastic. And the staff throughout the entire building were very friendly.

Despite all the praise, I must also mention a few points of criticism:

No nightly oxygen saturation check was performed.

The equipment training was clearly neglected.

The mattresses caused me significant back problems.

The room cleaning was completed in a few minutes.

No applications took place last week due to (justified) strike action.

All in all, the rehabilitation was very beneficial for me, and I can recommend the facility with the aforementioned limitations. It was my ninth pulmonary rehabilitation stay, so I think I have a good understanding of the various facilities.

According to the Federal Ministry of Health, "rehabilitation services" encompass all medical services that serve to prevent, eliminate, reduce or compensate for a disability or need for care, to prevent its worsening, or to mitigate its consequences.

Rehabilitation and preventive care services can be provided on an outpatient or inpatient basis; they can be provided at the place of residence (in the case of mobile rehabilitation, even in the home of the person to be rehabilitated or, for example, in a care facility), in a recognized rehabilitation clinic or in an inpatient preventive care facility.“

If only it were always as easy as it sounds. Locally? Who among you has a rehabilitation facility nearby that specializes in alpha-1 deficiency? Having one close to home would be a dream come true.

The Federal Ministry's website actually explains very well what rehabilitation means, what types of rehabilitation exist, and even describes topics such as reimbursement and the right to choose. The webinar (see page 37) with Ms. Scherhag and Prof. Koczulla also provided us with a great deal of knowledge and support. However, the real problem that we, as Alpha-1 patients, see, is hardly addressed. Therefore, we have launched a "Rehabilitation" project within the board of Alpha1 Germany.

We need you for this!

We need your experiences, your reports on rehabilitation clinics that we might not even be aware of. The Schön Clinic in Berchtesgaden, with Prof. Koczulla, is clearly a leader in research on alpha-1 antitrypsin deficiency in relation to rehabilitation and training. The results achieved there help us to better manage alpha-1 and implement more targeted training. However, we mustn't forget that everyone experiences rehabilitation differently, and it's essential to determine what works best for each individual. The many experiences of others are invaluable in this process and in deciding which rehabilitation clinic is right for you. We have therefore compiled some of these experiences in this journal. Furthermore, together with the German Respiratory Society/Alpha1 Centers, we have begun surveying rehabilitation clinics about their suitability for alpha-1 patients.

Our primary wish is that

Alpha-1 antitrypsin deficiency is well known in clinical practice.

The training is personalized and tailored to our specific condition.

Consideration is given to mental health, possibly with psychological support.

Training courses will be offered as needed (inhalation control, breathing-relieving positions, emergency procedures, where to get help, etc.)

Social welfare advice is available

Good cooperation during this time is ensured, which also includes areas such as care, food and accommodation.

And last but not least, it's crucial that substitution therapy is organized locally and can be continued without problems. We hear the most outlandish justifications in this area for why a facility doesn't offer this service, even though it's obligated to do so. Unfortunately, there are too few rehabilitation facilities that are familiar with alpha-1 deficiency. This needs to change. There are only one or two outpatient options nationwide (!), and it's high time this issue was addressed.

Please feel free to share your experiences with us. You don't have to write entire texts right away; we can also work with you to formulate them. If you wish, we can use your experiences anonymously in advising others, or you can share your experiences with other patients via our website or journal – we would be delighted if you would support us in making this section valuable for all Alphas.

The term physical activity describes what one actually does each day. Medical professionals understand capacity to mean what one is capable of doing or what one could physically accomplish. This doesn't mean that one actually does it. It is clear, however, that a lot of activity leads to better capacity. Medical rehabilitation aims to improve capacity in the long term. Maintaining good capacity is a lifelong task for everyone, and especially for those with chronic illnesses.

The influence of exercise on health and illness

How quickly a lack of physical activity affects your capacity becomes apparent when you've had to spend a few days in bed due to illness. When you get up for the first time afterward, your legs feel wobbly. You've lost muscle mass from lying down. In cases of severe illness, up to 1 kg of muscle can be lost per day. Considering that women have an average of about 30 kg and men around 35 kg of muscle mass, 1 kg is a considerable amount. The COVID-19 pandemic has shown that it can take a while to regain your previous level of performance.

While humans have continued to evolve in terms of thought and inventions, the amount of physical activity has decreased drastically: while Stone Age people covered distances of around 30 km per day, this figure had dropped to only about 15 km per day by the 19th century. Nowadays, most of our time is spent sitting, and many people walk only 1 km a day.

The World Health Organization (WHO) recommends two and a half hours of physical activity per week as a physical activity goal. In its October 2022 report, the WHO details how prevalent physical inactivity has become. Approximately 80 percent of adolescents and 60 percent of those over 70 are not physically active enough; the situation is slightly better for other adults, at 60 percent. Germany fares worse than its neighbors. In poorer countries, lack of exercise is a much less common problem than in wealthier ones. Furthermore, the average daily step count decreased by a third due to the COVID-19 pandemic. This, too, has contributed to the pandemic's negative impact on health.

A British study with 80,000 participants demonstrates the significant role physical activity plays in long-term health. Their daily step count was continuously recorded over seven days using a reliable fitness tracker (accelerometer). Seven years later, researchers analyzed how many individuals had died since the start of the study and the documented causes of death. A clear correlation emerged between mortality and the number of daily steps: the more steps taken, the lower the subsequent mortality rate. This held true for daily steps up to 10,000; beyond that, overall mortality remained roughly the same. High activity levels reduced mortality from both cardiovascular disease and cancer. A protective effect of regular exercise was also demonstrated for Alzheimer's disease.

Measuring movement

The first pedometer was developed in Japan in 1964. Even then, 10,000 steps per day were recommended. Nowadays, there are other ways to measure movement. Most cell phones have built-in pedometers. However, they can only fully record activity if the phone is constantly carried on the body. Fitness trackers or watches are therefore more informative. Using the number of steps taken, doctors determine the activity level; those who take fewer than 5,000 steps per day are considered "inactive.".

You can motivate yourself to be more active by setting clear goals. For example, being able to dress yourself again, resuming a former hobby, or having your clothes fit better. Having a dog is a great motivator to leave the house and go for a walk several times a day. Many seniors also look after their grandchildren and are more physically active doing so than when alone. Joining a pulmonary rehabilitation program and exercising with others also brings more activity into everyday life. Everyone should do what they enjoy and what is good for them. A training plan must be tailored to the individual's needs and adapted accordingly.

rehabilitation

A rehabilitation program encompasses several different facets. Endurance and strength training is just one aspect. Experts from various professional groups work with their patients in the areas of physiotherapy, relaxation, education, mental health, nutrition, and occupational therapy. The rehabilitation clinic also conducts comprehensive diagnostics to document the patient's current condition. Doctors and therapists also optimize medication and/or oxygen therapy.

The indications for pulmonary rehabilitation were defined in guidelines 10 years ago. These include chronic conditions such as shortness of breath or reduced exercise tolerance, but also a reduced quality of life or psychosocial problems that can arise as a result of the lung disease.

In some scientifically monitored rehabilitation programs, muscle tissue samples have been examined. These studies show that not only is the mass of muscle cells reduced, but this can also affect the function of the muscle fibers. Specifically in AATD (Alpha Type I Dysfunction), it seems difficult to increase the number of type I muscle fibers, responsible for endurance, through training. Rehabilitation has a greater effect on type IIA muscle fibers, which generate muscle strength. Researchers have found differences in the response of COPD and Alpha1 patients in studies, suggesting that the training program may need to be more precisely tailored. An individually customized training program can demonstrate convincing and lasting results, as illustrated by the case study of one patient. It is important to remain physically active after rehabilitation to prevent the progress made. Minimizing sitting and lying down in everyday life and increasing movement is beneficial for health.

Digital health applications (DiGA): The KAIA COPD app, developed specifically for COPD patients, runs on smartphones and tablets. It has been recognized as a DiGA by the Federal Institute for Drugs and Medical Devices and can be prescribed by a general practitioner. With the prescription, you contact your health insurance provider and receive a QR code. This code activates the app, which you download from the internet. The activation is valid for three months and must be renewed in the following quarter.

The KAIA COPD app has three pillars: physical training, breathing techniques, and information. While exercising, users can record themselves with their phone's camera. The programmed artificial intelligence recognizes whether the exercise is being performed correctly and provides feedback with recommendations if necessary. This works almost as well as being present with a sports therapist. Using diagnostic tests such as the stand-up test or the toe-reach test, the app assesses the user's fitness level at the beginning of the workout.

In a study, the KAIA COPD app was tested on patients discharged from pulmonary rehabilitation. Those who used the app for six months were able to maintain their step count at home, the same level they had reached at the end of their rehabilitation. In contrast, a control group experienced a significant drop in step count after six months, from 5,000 to 3,100 steps per day. Patients who used the app at least four times a week were even able to increase their step count by an additional 1,000 steps per day. The app-user group also demonstrated stable health-related quality of life, while the other patients experienced a decline. Thus, the KAIA COPD app has been proven to help maintain the positive effects of rehabilitation over six months. This digital COPD therapy via smartphone app provides a useful health companion for everyday life.

Telemedicine

In the USA, a telemedicine rehabilitation concept for COPD patients has been developed. There, the smartphone facilitates contact between the patient and the doctor or physiotherapist. The patient's exercises, including equipment training, are directly observed and commented on by the therapist.

The benefits of this program were tested on 80 COPD patients who had been hospitalized due to an exacerbation. After discharge, three telemedicine appointments per week were intended to prevent further exacerbations. In the first 30 days after discharge, 18 patients in the control group required readmission, while only 6 patients in the telemedicine training group required readmission. Here, too, the health of people with lung disease was stabilized through the use of modern digital technologies. A similar program, PneumoFactory, was developed in Germany.

Ultimately, every rehabilitation program should be individually tailored to the patient. Doctors and therapists suggest measures, and the patient decides what suits them best. Even as COPD medications become increasingly effective, non-pharmacological methods continue to make a significant contribution. Apps and telemedicine are new components that patients can incorporate into their individual programs.

Sara Souto Miranda, a doctoral student at the School of Health Sciences of the University of Aveiro (ESSUA) Portugal, brought the following online study to our attention:
Delphi survey for a core outcome set for pulmonary rehabilitation in patients with chronic obstructive pulmonary disease (COPD)

Goal

Help us define a core outcome set for the pulmonary rehabilitation of people with COPD by participating in a Delphi survey.

What is a core result set?

A core outcome set is an agreed-upon standardized set of outcomes that should be measured and reported at a minimum in all clinical trials in certain areas of health or healthcare.

Why do we need a core outcome set for pulmonary rehabilitation?

There is significant heterogeneity in the outcomes/effects and interventions reported for pulmonary rehabilitation in people with COPD. This complicates the comparison of studies and programs, effective evidence synthesis, and benchmarking of pulmonary rehabilitation.

Who can participate in the survey?

People with COPD:

have a diagnosis of COPD

Have participated in pulmonary rehabilitation

Informal caregivers

Adult

Informal relationship with a person with COPD (e.g., family member, friend, neighbor)

Support (physical, emotional, financial, logistical) for a person with COPD who has participated in pulmonary rehabilitation.

Healthcare professionals, researchers, political decision-makers

Adult

Involved in the conception, implementation and/or evaluation of pulmonary rehabilitation measures or their data.

Course of the disease

The COVID-19 disease, caused by the novel SARS-CoV-2 virus, originated in China and spread to Europe. Since early/mid-March, the pandemic has also been unfolding in Germany, with severe, sometimes unexpected, cases and fatalities.

Besides asymptomatic infections, mild to moderate cases are observed, but also very severe cases that lead to hospitalization and intensive care. Unfortunately, fatalities also occur.

In approximately 801 of the registered infections, the disease presents with fever or mild symptoms. In about 151 of the cases, it is more severe, and in 51 of the cases, it is so severe that intensive care is necessary because the condition is considered critical or life-threatening.

Bilateral pneumonia with respiratory failure is a key pathology in severe cases. To date, there are no data specifically describing the course of the disease in Alpha-1 patients.

Proof

Diagnosis is made via a deep nasal/pharyngeal swab. We assume an incubation period of 14 days. Typical symptoms are fever, cough, and shortness of breath. However, asymptomatic cases are also possible. Diarrhea may occur, as well as changes in the sense of smell and taste. Shortness of breath, fatigue, and muscle pain can be further typical symptoms. In addition to lung involvement, cardiovascular involvement has been described. Neurological changes have been reported, including ischemic stroke, thromboembolic events, meningitis, headaches, altered mental status, and critical illness polyneuropathy following a severe hospital stay.

prophylaxis

For preventative measures, the recommendations issued by the Robert Koch Institute apply. Maintaining distance, wearing a face mask, and regular hand disinfection are important and can be found in all recommendations.

Therapy in the acute phase

Currently, there is neither a specific treatment for COVID-19 nor a vaccine. Besides symptomatic treatment—that is, administering oxygen when oxygen saturation drops and blood oxygen levels are low—treatment options include mechanical ventilation and intensive care for severe cases. There are some studies on medications—such as the antiviral drug Remdesivir, which can shorten the course of the disease by a few days.

Post-acute phase

How should someone recovering from a COVID-19 infection be cared for afterward? A prompt follow-up appointment with a pulmonologist is certainly advisable, where lung function tests, blood gas analysis, and diffusion capacity measurements could be useful. If heart problems are present, a cardiac examination should also be performed. In cases of severe impairment, imaging in the form of a high-resolution CT scan may be beneficial. Further diagnostic procedures should be determined based on the CT findings.
According to current understanding, Alpha-1 ATG administration should not be interrupted if possible. The research group led by McElvaney was able to show (New England Journal 2020) that interrupting alpha-1 AT therapy leads to an increased exacerbation rate.
It is assumed that everyday physical activities such as walking and moderate physical training are beneficial in the post-acute phase.

Inpatient rehabilitation

Furthermore, the care provided after mild to severe cases of COVID-19 is certainly an important consideration. As a direct consequence of COVID-19, it is assumed that the number of patients with long-term health consequences related to COVID-19 will increase significantly. While patients with asymptomatic cases appear to recover completely and without long-term limitations, initial studies show that mild, severe, and critical cases of the disease have lasting effects in various areas (cognitive function, skeletal muscles, mental health, etc.). Clinical observations indicate that COVID-19 is primarily characterized by lung damage and can thus lead to persistent shortness of breath. Impairments in blood clotting are also observed, which can sometimes lead to the occlusion of smaller vessels or even to a pulmonary embolism. The specific consequences of COVID-19 in patients with a pre-existing lung condition such as alpha-1 antitrypsin deficiency are currently largely unknown. It is assumed that the long-term effects of the disease can be addressed and reduced through specialized rehabilitation. Data from the 2003 SARS-CoV-1 outbreak show that rehabilitation can positively influence the post-acute course of the disease. The content of pulmonary rehabilitation (PR) in the post-acute COVID-19 phase is based on established pulmonary rehabilitation programs for chronic lung diseases. In the international PR guidelines of the American Thoracic Society (ATS) and the European Respiratory Society (ERS), PR has become established as a core component of disease management for patients with chronic respiratory diseases. Studies demonstrate clear positive effects on physical performance, quality of life, and symptoms (shortness of breath, fatigue).

A Swiss group of authors proposed using the same rehabilitation treatments recommended for patients with pulmonary fibrosis, as the observed pulmonary and extrapulmonary symptoms are very similar (Carda 2020, Kenn 2013). This comprehensive, multimodal, and multidisciplinary therapeutic approach combines both pharmacological and non-pharmacological therapies. In addition to optimizing medical care, a targeted exercise program is a core component. Physical training in pulmonary rehabilitation (PR) consists of at least individually tailored endurance and strength training, which should be conducted with a mobile oxygen supply in cases of severely reduced blood oxygen saturation. Another component of PR is respiratory physiotherapy, which aims to restore respiratory function as much as possible and to teach symptom-reducing breathing techniques for both rest and exertion. Furthermore, psychological support and knowledge transfer are important components of PR. The extent to which these components are also optimal in patients who have experienced COVID-19, or whether they can be further improved, will be determined in clinical practice over the coming weeks and in further studies.
to reveal on this topic.

Naturally, the COVID-19 situation has significantly changed the procedures in our hospital. In the first weeks after the outbreak of the pandemic up to the present day, we have actively helped care for COVID-19 patients.
We resumed routine operations at the end of May, but we continue to keep rooms physically separate for COVID-19 patients. We have implemented special hygiene measures in our daily routines, as prescribed by the Robert Koch Institute (RKI). A dedicated team is available for COVID-19 patients.

We have also begun developing a concept for post-COVID-19 patients, which is intended to meet the needs of this specific patient group and is also being addressed within the framework of studies. In general, we have meticulously implemented the hygiene regulations of the Robert Koch Institute (RKI) throughout the hospital. With the easing of the corresponding social distancing measures across Germany, there is, of course, a general risk of reinfections occurring, which could lead to a renewed exacerbation of the COVID-19 situation in hospitals as well.

The primary symptom of COPD is shortness of breath. This is not only very unpleasant, but also sets off a downward spiral: because of the shortness of breath, people avoid physical exertion, which impairs endurance and muscle strength, leading to a decrease in fitness, which in turn exacerbates the shortness of breath during the next physical activity. The shortness of breath itself also triggers feelings of anxiety. On a psychosocial level, shortness of breath has an impact, for example, by limiting one's ability to participate in activities with family and friends.
COPD is now considered a disease that is not limited to the lungs. Rather, other areas of the body are also affected, especially the cardiovascular system, but also metabolism and bones. And not least, the muscles, as current research shows. In COPD, the skeletal muscles themselves are altered by chronic inflammatory processes, oxygen deficiency, a negative energy balance, or even adverse effects of medications.

Pulmonary rehabilitation as a comprehensive treatment

The downward spiral can be broken with pulmonary rehabilitation. Today's stays in specialized lung clinics, lasting at least three weeks, have nothing in common with the spa treatments of the past. Rather, rehabilitation is based on modern scientific findings published in treatment guidelines by international experts. During rehabilitation, therapists from various professional groups work together to design the optimal rehabilitation program for each individual patient. The essential components are physical training, education, and motivation for behavioral change.
Medical diagnostics include comprehensive lung function tests and blood work. Physicians inquire about current medications and ensure that inhaled medications are being used correctly. Long-term oxygen therapy and non-invasive ventilation are also checked for optimal effectiveness. Information and education on medical facts are crucial to empowering patients to become experts in managing their own condition. Rehabilitation encourages patients to make positive lifestyle changes. Psychological support is available to address and treat anxieties.
Physiotherapy and exercise therapy aim to improve the patient's endurance, strength, and coordination. The focus is on activities the patient can participate in daily life, such as climbing stairs at home.

Effectiveness of pulmonary rehabilitation in COPD

Rehabilitation is the most effective treatment for alleviating shortness of breath, improving physical performance, and enhancing the patient's quality of life. COPD patients of all severity levels benefit from this. The greatest effects are achieved in moderate to severe COPD. Scientists have summarized and evaluated research findings on rehabilitation from numerous studies. Regarding quality of life, patients experienced noticeable improvements in fatigue, mood, coping with the illness, and shortness of breath. Physical performance is frequently measured using the 6-minute walk test. After rehabilitation, the average walking distance improved by 44 meters, which is a very good and important result for the patient. These figures refer to a comparison between rehabilitation and a control group that did not receive rehabilitation.

When patients received pulmonary rehabilitation immediately after a COPD exacerbation, their 6-minute walk distance increased by an average of 62 meters more than in the control group. Quality of life after an exacerbation also improved significantly following rehabilitation. An equally important finding was that fewer patients required readmission to the hospital during the follow-up period if they had participated in a rehabilitation program. When comprehensive rehabilitation was implemented, in addition to physical training, mortality was also lower than in the control group.

Differences between COPD and AATM

Although AATD is classified as a chronic obstructive pulmonary disease, there are crucial differences: AATD patients are usually younger, can develop severe lung disease despite not smoking, and are much more likely to suffer from bronchiectasis than COPD patients. AATD, unlike COPD, is caused by genetic alterations. And only AATD benefits from alpha-1 antitrypsin replacement therapy.
Since 2013, international guidelines for pulmonary rehabilitation have specifically recommended pulmonary rehabilitation for patients with AATD who also have COPD. However, until then, there were only a few studies specifically on the effects of training in AATD patients. A research group from the Schön Clinic Berchtesgadener Land is attempting to fill this gap. An analysis of medical records showed that COPD patients improved their 6-minute walk distance by an average of 62 meters after rehabilitation, while the improvement for AATD patients was only 48 meters. This suggests that the adaptation processes after training may differ between AATD patients and COPD patients without AATD.

Effects of physical training in COPD and AATM

For a comprehensive training study, participants agreed to have a small piece of muscle tissue taken from their thigh at the beginning and end of their rehabilitation. Strength and endurance were also documented using stress tests, and the patient's maximum performance was recorded. This was followed by a 21-day inpatient training program with five training sessions per week using the interval method. This method alternates short, very intense bursts of exertion with rest periods. During each 30-second burst, the patient exerts themselves at their maximum capacity, followed by a 30-second rest. This cycle is repeated 10-20 times. Such interval training can be very effectively adapted to the individual capabilities of the patient, even in the case of seriously ill patients. The patient records their shortness of breath and leg fatigue in a training log.

This training study was evaluated in nine individuals with AATD-related COPD and 10 COPD patients without AATD. They already had significantly reduced lung function values, with an average FEV1 around 301 Tp3T of predicted. Their daily physical activity was comparable to that of truck drivers. Through training and rehabilitation, physical performance increased by an average of nine watts in the AATD patients, representing a relevant improvement. However, the increase in COPD patients was even more pronounced, reaching 18 watts. The analysis of the muscle biopsies was particularly interesting. Muscle tissue contains different types of muscle fibers, such as type 1 fibers, which are crucial for endurance, and type 2a fibers, which enable short bursts of high-intensity exercise. In healthy individuals, it is expected that type 1 fibers would increase after a training program. Surprisingly, the study patients with AATD showed no increase in type 1 muscle fibers after the three-week training period, but only in the more strength-oriented type 2a fibers. Researchers found a different picture in COPD patients. Their muscle samples more closely resembled those of healthy individuals, as they had more type 1 fibers after training than at the beginning. The reasons for these differences between the patient groups are not yet clear. However, it could be that people with AATD benefit more from a different type of training.
The research group at Schön Klinik Berchtesgadener Land is starting a new study on this topic and is still recruiting AATM patients! They should have a PiZZ type, their FEV1 must be below 50% of predicted, and they must not have diabetes mellitus. The study procedure will be similar to the previous training study, meaning muscle tissue will also be examined.

Active lifestyle at home

A weakness of rehabilitation arises during the transition to the home environment. If one has benefited from rehabilitation, how can this effect be maintained in everyday life at home? Ideally, the patient would continue to participate in an outpatient exercise program, such as pulmonary rehabilitation or medical training therapy. However, doctors cannot simply prescribe exercise and training as easily as medication.

It's important for everyone to become more active in their daily lives. This means sitting less, not being afraid to climb stairs, even if slowly, and going for frequent walks or staying mobile in other ways. For COPD, it's well-established that an active lifestyle significantly extends lifespan, and a similar effect can be expected for patients with AATD – although, as is unfortunately so often the case, there are still no AATD-specific studies on this.

Summary: Prof. Dr. med. Gratiana Steinkamp, as published in Alpha1 Journal 1-2019.

Numerous scientific studies – including many from the Schön Clinic Berchtesgadener Land – demonstrate that patients with lung and respiratory diseases can benefit enormously from rehabilitative approaches. Especially for patients with chronic obstructive pulmonary disease (COPD), physical training is among the most effective measures and sometimes achieves greater success than purely drug-based therapies. Patients regain physical fitness, can expand their range of activities, and experience a return to a better quality of life. The Schön Clinic Berchtesgadener Land (BGL) is a nationally recognized specialist rehabilitation clinic with the medical focus areas of orthopedics, psychosomatics, and pulmonology.

The Center for Pneumology, Allergology, Sleep Medicine, and Respiratory Care is headed by Prof. Andreas Rembert Koczulla, who took over the position of Chief Physician from Prof. Dr. Klaus Kenn in August 2018 and, according to the current Focus Rehabilitation List 2019, has been recognized for top medicine, top quality, and top rehabilitation clinic status. Prof. Koczulla and Prof. Kenn hold Germany's only professorship for pulmonary rehabilitation. As a research institution of Philipps University of Marburg, the clinic maintains close ties with international lung centers and is committed to continuously developing pulmonary rehabilitation concepts and directly transferring this expertise to patients. The primary goal is not only to ease breathing but, above all, to improve the quality of life and psychological well-being (e.g., anxiety about breathlessness) of those affected.

The most important indications at a glance:

Chronic obstructive bronchitis with/without pulmonary emphysema (COPD)

Alpha-1-antitrypsin deficiency (AATM)

asthma

Vocal Cord Dysfunction

Pulmonary fibrosis

Sleep-related breathing disorders

Rare lung diseases

Condition after lung transplant

The Schön Clinic Berchtesgadener is Europe's largest center for the pre- and post-operative care of lung transplant recipients. Furthermore, as an Alpha-1 Center, the clinic provides care for patients with congenital AATD. Due to the limited options for drug treatment, non-drug therapies, and in particular exercise therapy, are of paramount importance. Professor Koczulla and his team possess extensive experience and specialized expertise in treating this condition.

In addition to medication adjustment and substitution therapy, a regular training The most important component of therapy in the treatment of patients with alpha-1 antitrypsin deficiency. This is confirmed by current studies.

Many terms from training theory – reaction speed, anaerobic or aerobic endurance, strength endurance, coordination, explosive or speed strength, hyperflexibility, action speed, reactive strength, speed endurance, maximum strength – are merely theoretical concepts. These are not helpful in overcoming inertia and motivating exercise. The following reasons are far more relevant to everyday life and explain why patients with alpha-1 antitrypsin deficiency should exercise despite shortness of breath and lack of motivation:

increases lung volume

improves breathing

improves oxygen uptake and transport

slows down the brain aging process (dementia protection)

Increases insulin sensitivity (reduces type 2 diabetes)

trains coordination (fall prevention and prophylaxis)

Increases bone density (osteoporosis prophylaxis)

lowers blood pressure

promotes physical well-being

strengthens the immune system

offers social contacts

improves quality of life

increases mobility

Don't forget performance-limiting factors.

However, factors that limit performance should not be overlooked. The Posturomed has long been established in proprioceptive (deep sensory) training for respiratory patients. Here, under the guidance of therapists, the strain is trained in combination with breathing using intrinsic vibrations and equipment. The "Galileo" device is being used more and more frequently for respiratory patients – and therefore also for those with alpha syndrome. Recent studies confirm this. With Galileo, the training frequency determines the number of stretch reflexes per second. This therapy offers increased muscle work with a reduced cardiovascular response. This stimulates metabolism and blood circulation. However, those suffering from diabetes should be aware of the risk of hypoglycemia. Depending on the training frequency setting, Galileo training serves to relax/detonize muscles, improve muscle function/coordination, or increase muscle performance and strength.

The training goal determines the frequency.

It's important to note that the frequency is selected based on the training goal. However, Galileo training is only effective and leads to targeted performance improvements if the exercises are performed consciously and, above all, actively. New scientific findings also recommend training the respiratory muscles. Due to the significant weakness of the respiratory muscles (detected by PImax measurement during lung function testing and a defined training zone established by a physician), it initially seems contradictory to further train an already overworked respiratory system.
To further stress the muscles through additional training. However, due to the training baseline, an effect will only become noticeable after approximately 14 days of consistent and regular training.

The threshold load method is well suited for patients with alpha-1 antitrypsin deficiency.

For patients with chronic respiratory disease – specifically, those with alpha-1 antitrypsin deficiency – the threshold load method has proven ideal. A predefined and set threshold (30 % of the PImax value) creates negative pressure. As soon as this threshold is exceeded during inhalation, the valve releases, allowing air to flow in. However, proper instruction by a therapist during supervised sessions and correct execution of the training are crucial. Each training session consists of seven 2-minute intervals (with a maximum of 20-30 breaths per interval).

An important component: Ergometer training

Ergometer training is also an important component of therapy – both as part of independent training on a home ergometer and as part of supervised and computer-controlled training during pulmonary rehabilitation. Here, the training can be used as
Interval training can be performed. This is particularly suitable for patients who need to be gradually reintroduced to physical exertion after a prolonged hospital stay. Constant training – in addition to heart rate-based or SO2-based training – is also possible with newer systems as part of rehabilitation. For medical training therapy (MTT) or as an extension of pulmonary rehabilitation, there are various prerequisites that are beneficial for an Alpha-1 patient:

Individual settings for each patient

(Mobile) control mirrors to prevent protective behavior or use of auxiliary respiratory muscles and to check upright posture

Footrests to help maintain basic physical tension more easily

Smaller weight increments are better for patients with severe respiratory illnesses.

Additional weights allow for better adjustment to the actual performance level.

Pre-tensioners eliminate the problem of a slack path.

Training isolation: A one-sided isometric training program to reduce cardiovascular stress after a prolonged hospital stay and to gradually reintroduce physical exertion.

The Borg scale (see image) is always a suitable tool for training control across all types of training. The optimal training range is between 4 and 6. Training for everyday life is a primary therapeutic goal in the collaborative work between patients and therapists. This applies to both rehabilitation and pulmonary exercise programs. Training programs tailored to the individual's physical capacity are developed collaboratively, with the aim of improving daily life management. A realistic target is crucial and must be regularly adjusted. Integration and feasibility into daily life must also be considered to achieve consistent training.

Within the framework of training management, this means clarifying the patient's difficulties and problems in their daily life. The spatial conditions, personal preferences and expectations, as well as the patient's willingness to participate, must also be taken into account. This leads to the creation of an individually tailored training plan. This plan includes, among other things, practicing movement patterns that are difficult due to illness. Certain movements are adapted and broken down into individual movement components. The goal of the training is to gradually increase the initial speed of each exercise.

The Borg scale, ideally displayed at several points in the training room, allows for the assessment of training progress – starting with the warm-up phase and continuing through the subsequent build-up phase. This helps identify the optimal training zone for each individual. Performance is improved, and the intensity is gradually increased. The gradual increase in workload should be monitored by repeatedly recording the breathing rate (no more than 4 breaths in 10 seconds). This enables the correct selection of the exercise intensity. Prior to this, comprehensive instruction in the "pursed-lip" breathing technique and various breathing-facilitating body positions is essential.
