	<p><strong>My name is Katrin Heimann, I am PiZZ, I have been receiving opioid substitution therapy for 13 years and I use an oxygen concentrator during physical exertion.<br />
At the beginning of 2025, I received the news that I would be allowed to participate in a rehabilitation program in October at the rehabilitation clinic in the seaside resort of Ahlbeck on Usedom. </strong></p>
<p>I&#039;d like to tell you about this stay. I won&#039;t go into the application process, deadlines, etc., because my pension isn&#039;t covered by the German statutory pension insurance, but by a professional pension scheme. This means the organization and the support I can expect are very individualized.</p>
<p>The clinic wanted me to be admitted in the morning, which, given my 450 km journey, meant that I checked into a campsite the evening before.</p>
<p>I was greeted very kindly at reception the next morning, seated in the dining room, then picked up by a colleague from the ward, and quickly had my room, an appointment with the doctor, and my first lunch.</p>
<p>The clinic may not be furnished in the very latest style, but it feels welcoming and well-organized from the moment you arrive. The light-filled dining room, the attractively furnished rooms with ample and well-designed storage space, and the sound of the Baltic Sea, which is wonderfully audible in the room, made my start very easy. My therapy was determined by the doctor on the first day, and so it began the following morning.</p>
<p>Medical training therapy, breathing exercises on the beach, coordination training, ergometer training, theraband exercises and water aerobics in the hotel&#039;s own very nice swimming pool were combined in a good mix with mud packs, massage therapies and inhalations over the following days and really relaxed me.</p>
<p>Furthermore, I received intensive training on COPD. At first, I thought: YES, I already know all that… But the training was really well done, and I didn&#039;t feel like I was wasting my time.</p>

	<p>Our condition was only mentioned in passing… but that didn&#039;t bother me. For specific information, there&#039;s the association (which I did a little advertising for).</p>
<p>Fortunately, I was even able to extend my stay by a week. The organization was a bit bumpy at first, but everything worked out in the end.</p>
<p>If I have any criticism at all, it&#039;s that the adjustments to the therapies were sometimes implemented with a slight delay. I think that&#039;s hardly surprising in clinics of this size. As a patient, I quickly felt like time was running out. Ultimately, everything always worked out in the end. My advice to everyone is to take initiative, whether by asking questions or by actively shaping your own free time (e.g., through PROMISE training sessions... another topic... but also great). When I had problems, I simply asked the therapists about them and always found them receptive.</p>
<p>The ladies who arrange the seating in the dining hall have a real knack for it. I truly felt like there were benevolent spirits at work. I had incredibly nice people at my table. Looking at the surrounding tables each day, I got the impression of a general sense of contentment. There was laughter and chatter; the illnesses were grouped across various medical specialties, which pleasantly helped to fade into the background.</p>
<p>Some evenings I ended the day in the Viennese café belonging to the clinic, which was open to the public; on other days I was able to go for a short bike ride.</p>
<p>Describing the incredible beauty of the Imperial Spa architecture, the wonderful feeling of &quot;just hopping on your bike&quot; to Poland, or the beauty of the hinterland of the sunny island of Usedom would be beyond the scope of this article. &quot;Take the waters where others vacation&quot; says it all.</p>
<p>I was very satisfied with my stay at the rehabilitation clinic in the seaside resort of Ahlbeck.</p>

	<p><strong>Swedish Coffee Klatch – Baking without milk, eggs and nuts is far more than a classic baking book. </strong></p>
<p>It is aimed at people who often experience limitations when baking due to allergies or intolerances – as well as all those who have guests with such needs and still don&#039;t want to forgo delicious treats. With around forty lovingly developed recipes, the book impressively demonstrates that cakes, cookies, and desserts can be delicious even without milk, eggs, and nuts, and that they are in no way inferior to traditional baked goods in terms of texture and appearance.</p>
<p>These recipes are the result of years of experimentation, testing, and refinement. They clearly demonstrate how diverse and creative low-allergen baking can be – from simple everyday pastries to special treats for a cozy coffee gathering. This typically Swedish way of life, enjoying coffee together, is made accessible to everyone here, regardless of food allergies.</p>
<p>What is particularly noteworthy is the person behind the book: The author, Mikaela Odemyr, has been actively involved with the European Lung Foundation (ELF) for many years and, as head of the Swedish Allergy and Asthma Association, advocates for people with respiratory and allergic diseases at the European level. I met her personally through my work and was immediately impressed by her dynamism, energy, and tireless dedication. This passion is clearly reflected in her book. Her daughter, photographer Lisa Odemyr, studied journalism and communication science in Vienna and now works as a journalist.</p>
<p><strong>Swedish coffee klatch</strong> It therefore stands not only for creative recipes, but also for quality of life and participation. It shows that conscious nutrition, consideration of health needs, and genuine enjoyment don&#039;t have to be contradictory – but rather, in the best case, mutually enriching.</p>

	<p><strong>On the initiative of the chief physician of the internal medicine department of St. Marien-Hospital Cologne, Dr. Andreas Schlesinger, a presentation of the outpatient rehabilitation center AmKaRe took place in Cologne-Poll on October 22, 2025. </strong></p>
<p>All interested patients as well as the heads of the sarcoidosis network, the pulmonary fibrosis association and the self-help group Alpha1 Cologne were invited to the rehabilitation center.</p>
<p>The rehabilitation center was first presented by its managing director, Heike Jeß. AmKaRe is an outpatient rehabilitation center for heart, lung, and vascular conditions in Cologne. Each rehabilitation program lasts three weeks and is conducted full-day, five days a week. A pick-up service from home is offered upon request.</p>
<p>There is generally no difference between applying for inpatient or outpatient rehabilitation. AmKaRe is happy to assist with the application to the pension insurance provider or health insurance company. At the beginning of the rehabilitation program, a medical consultation and examination by a cardiologist or pulmonologist will take place.</p>
<p>The rehabilitation program itself includes a wide range of services. A team of doctors, sports scientists, psychologists, and nutritionists compiles the services and provides support throughout the rehabilitation process.</p>
<p>After completing rehabilitation, an affiliated physiotherapy practice is available where you can continue training or receive supplementary treatments with or without a prescription.</p>
<p>Of course, physical well-being is also taken care of during the rehabilitation.</p>
<p>After the presentation by AmKaRe, our self-help groups were given the opportunity to introduce themselves. The respective group leaders presented their work. Afterwards, there was an opportunity for personal exchange. A small buffet was also provided.</p>
<p>I was able to have interesting conversations with Mrs. Jeß, Dr. Schlesinger, physiotherapists and the other self-help groups.</p>
<p>Overall, it was a very intensive afternoon with new and interesting contacts. Regular contact was agreed upon with all participants.</p>
<p>I think the idea of considering outpatient rehabilitation as well as inpatient care is fantastic. Of course, AmKaRe is primarily suitable for patients who live in Cologne or the surrounding area. But hopefully, there will soon be many more outpatient rehabilitation providers in other regions.</p>
<p>Overall, I really liked the atmosphere at AmKaRe. Even the entrance to the park-like grounds is impressive. The overall concept, with the experienced pulmonologist and Alpha-1 expert Dr. Justus de Zeeuw, seems ideal for us Alpha-1 patients.</p>

	<p><strong>Having become curious about this small musical instrument, the Kazoo, I ordered two versions from a music store, one made of plastic and one made of metal, which I would like to compare here.</strong></p>
<p>The plastic kazoo, which I ordered in Alpha1 blue, is incredibly cheap at €1.90. The hole for singing into is quite large, and I find it a little difficult to close my lips tightly around it. Instinctively, I blow into it, and nothing happens! Of course, since I have to produce sounds for the device to start vibrating. A simple &quot;lalala&quot; sounds quite amusing and intentionally distorted. I fervently try to sing &quot;Shine on you crazy diamond&quot; and am surprised by how much concentration and gentle pressure I have to exert to make it produce a sound. My lungs are definitely getting a pleasant workout.</p>
<p>The metal kazoo, attractively designed in silver and gold and available for €4.90, has an oval opening, which I find much easier to use, as my lips close around it more easily. My Crazy Diamond kazoo sounds much more &quot;crazy&quot; on it, and I really like the version of &quot;The Cuckoo and the Donkey.&quot; It sounds even more interesting and distorted than the plastic version, and my lungs get a good workout from the small, rhythmic pulses. My impression is that it definitely makes sense to buy such a small training device if you want to challenge and strengthen your lungs with gentle, rhythmic pulses. The cool thing about it is that it doesn&#039;t matter whether you can sing well or not, because the sounds are intentionally distorted anyway. Personally, I prefer the metal kazoo; I find it to be of higher quality and also more comfortable to use. The music store recommends the plastic one as a musical instrument for children.</p>
<p>If you don&#039;t have a music store nearby, you&#039;ll also have to factor in shipping costs. How about purchasing it together, for example, as part of a lung exercise group, to save on postage? I can easily imagine it being used there, just as much as at home. Here&#039;s to some happy singing!</p>

	<p>The headline begins, so to speak, at the end of the story. But let&#039;s start at the beginning. Thanks to the fantastic organization of our self-help group here in the north, a patient day took place in Hamburg on September 21, 2024.</p>
<p>A key focus was the lecture by Prof. Dr. med. Ralf Eberhardt from the Asklepios Clinic in Hamburg-Barmbek. His lecture was on &quot;Interventional Bronchoscopy – Bronchoscopic Valve Implantation.&quot; It sounds complicated, but Prof. Eberhardt&#039;s presentation made it understandable and clearer to me. There was also time for informal discussions, and I decided that very day to pursue this topic further.</p>
<p>In October 2024, I had an appointment with my pulmonologist in Hamburg, and he supported my plan. So I received a referral, and both my doctor and I contacted Professor Eberhardt. He called me very quickly, and we scheduled an appointment for the inpatient preliminary examinations in November 2024. I stayed there for three nights, and many tests were performed, including a CT scan and a bronchoscopy.</p>
<p>It became clear here (as the professor had apparently already observed in several Alpha patients) that my right middle lobe was particularly affected. This area was therefore identified as the target for valve implantation. However, I still had to wait for a specific evaluation of the CT scan, and subsequently, I was sent to the Asklepios Clinic in Hamburg-Harburg for a special nuclear medicine examination.</p>
<p>Once everything was gathered and I apparently had no tissue bridges from the middle lobe to any of the other lung lobes, Professor Eberhardt called me again; this time he could offer me the procedure. I gladly accepted, and we scheduled an appointment for mid-February 2025.</p>

	<p>So the day finally arrived. After some examinations on the first day in the hospital, the procedure was performed by Professor Eberhardt himself the following day – even under general anesthesia. The procedure only took a few minutes, and I was quickly awake and physically fit again afterward. The dosage of the anesthesia is apparently very well adjusted to the brevity of the procedure. Thus, I was soon back in my room. On that day and the next, I had slight difficulty swallowing, probably from the tube, but it really wasn&#039;t a problem. A short time later, Professor Eberhardt visited me. I heard that everything had gone well and that I now had a larger valve in front of the middle lobe. Lying on my back, I could already feel that I hadn&#039;t been breathing in that area before.</p>
<p>The more active I became again, the more I started to feel a slight difference. The procedure had taken place on Wednesday, and a follow-up X-ray was done that same day. On Friday, I had another X-ray and also my first lung function test since the procedure. The X-rays showed that the middle lobe had shrunk, and the test also showed somewhat better hyperinflation values. Due to the fairly high risk of a pneumothorax after the procedure (because the other lungs have more space and can expand), I stayed in the hospital for another three nights. But everything went smoothly.</p>
<p>So, back to everyday life to see if and how things had changed. I quickly noticed that I felt, and still feel, somewhat better. Of course, the lung is still damaged, just like before, but by separating the most severely damaged area and thus shifting the focus to the rest, I feel better. I have a bit more stamina and recover much faster from shortness of breath than before. My partner also always says that we now have to stop for breather far less often when we go for walks together. It&#039;s the little things, and overall, I&#039;m glad I went down this path. Initial follow-up examinations with the pulmonologist and in March at Barmbek Hospital with Prof. Eberhardt confirm this. As I said, no fundamental change. For example, I walked the exact same distance in the 6-minute walk test before and after the procedure. It hasn&#039;t increased. But it was a bit easier afterward, and the lung function tests continue to show a noticeable improvement in hyperinflation values.</p>
<p>Sure, for example, the FEV1 is almost unchanged, but that was to be expected in my view. The next examination with Prof. Eberhardt will be in May, then also with a CT scan. The time after the surgery is still a process; it&#039;s quite possible that it will even improve.</p>
<p>I can only say that for me it was the right decision to achieve a noticeable effect through this ultimately minor endoscopic procedure. Thank you to Prof. Eberhardt and thank you to our support group for this important impetus.</p>

	<p>My name is Christine Stukan, I am 58 years old and have two daughters, aged 25 and 22. I am now fully retired and have had a lung transplant for almost exactly a year. I am feeling better than I have in decades, so I am happy to be able to tell you my story today.</p>
<p>I have experienced almost all facets and stages of COPD – from diagnosis to lung transplant. I want to encourage you and give you confidence that perseverance and fighting on will always be worthwhile; for us and for our loved ones.</p>
<p>In the early 2000s, I was referred to a pulmonologist by my family doctor because I was experiencing significantly more difficulty breathing and shortness of breath than usual. It took another six and a half years until, in 2009, I received the diagnosis of AAT genotype PiZZ and began Prolastin replacement therapy. For this, I was admitted to the University Hospital in Munich-Großhadern. I shared a room with another Alpha1 patient who was there on the lung transplant waiting list. Up until then, I had rather naively thought that I was missing an enzyme and that once I received the replacement, everything would be fine. Through my fellow patient&#039;s story, I was forced to grasp the full extent of the potential consequences of the disease.</p>

	<p>Over the years, my lungs deteriorated progressively. My daily life, juggling work, two school-aged children, and household chores, became increasingly difficult. I worked in an office, which is why it was still manageable. In 2011, during rehabilitation, I was prescribed long-term oxygen therapy. My Spirit (portable oxygen concentrator) and I became inseparable. With oxygen, I was actually able to exert myself considerably more. As a result, I applied for a higher disability rating and received the additional designation &quot;aG&quot; (extraordinary walking disability), which granted me the blue parking permit for severely disabled individuals – a significant improvement in my daily life. I struggled through life, which became increasingly difficult. Then, on top of everything else, I also had to organize and care for our elderly parents. I was barely managing to get by with all my responsibilities, but I had to make it work, and my job was very important to me and provided me with stability. Due to my now dire health situation, the topic of transplantation as a possible option came back into focus.</p>

	<p>In 2013, I went to rehab. I wanted to delve deeply into the topic of transplantation. Since it involves a highly invasive procedure with significant risks, it was a very difficult decision with far-reaching consequences for me and my family. After careful consideration, however, I initially decided against it because of the risks and because my growing daughters still needed me. In 2018, I had four valves implanted in the lower lobe of my left lung. These worked exceptionally well, and my lung function improved considerably. For a few weeks, I had a completely new quality of life and could breathe properly again. Unfortunately, this was short-lived because the elasticity of my lungs decreased, which can apparently happen in rare cases. This exhilarating feeling of being able to breathe properly again and get enough air motivated me to finally put myself on the lung transplant waiting list. I wanted to experience this quality of life again if I had the chance – despite all the risks.</p>

	<p>In November, I spent two weeks at the university hospital in Munich-Großhadern for a pre-employment examination. This meant a complete and thorough check-up. Given my already very poor health, this was really stressful and exhausting.</p>
<p>My current state of health was assessed, and cancer was ruled out. A psychologist also evaluated whether I was mentally stable enough for this major and stressful surgery. She wanted to know if I would be able to adequately and sustainably care for myself and a new lung, and whether I could accept and appreciate a new organ. The consultations with the surgical and anesthesiology teams also took place within this framework, so that they could react quickly if a donor organ were found. Thus, in February 2019, I was placed on the transplant list, and it was a matter of waiting for an organ. My mental health deteriorated, and to make matters worse, I developed anxiety and panic attacks, which had a truly dramatic impact. I had up to ten panic attacks a day, which left me so incapacitated that I could barely breathe. It was so bad for me that I sought help at the respiratory emergency clinic.</p>

	<p>It&#039;s part of the palliative care unit at the university hospital. They work with a completely different approach there: the shortness of breath originates in the lungs, but is triggered in the pain center of the brain. Low doses of morphine can improve the breathing somewhat. I also received help there through conversations with a psychologist, who subsequently recommended cognitive behavioral therapy.</p>
<p>In 2020/2021, I therefore began therapy to regain control over my psychologically distressing situation and to get the very debilitating anxiety and panic attacks under control. However, this only worked to a limited extent, since there was a physical cause for them. At least I learned to regulate myself in such situations so that I could breathe again and get enough air.</p>
<p>My health deteriorated steadily. As a result, I became increasingly fragile and hardly dared to do anything. Even the slightest exertion, like putting on socks or shoes, left me acutely short of breath. A cold or any other infection was a complete disaster for me. I could barely get through the day, even though I had nothing else to do but take care of myself. During this time, my daughter, who still lived at home, provided me with unwavering support, for which I am incredibly grateful.</p>
<p>During rehabilitation in 2021, I had to realize that I had to end my beloved job and could no longer live alone. This was a very difficult realization for me, as I had loved my independent life so much.<br />
I had. I then applied for care level 2 through the local social services and subsequently submitted an application for a disability pension after the rehabilitation.</p>
<p>In the spring of 2022, I moved from Munich to Weinheim to live with my sister and brother-in-law. They had made me the incredibly generous offer to live with them, even though my sister herself was seriously ill. My heartfelt thanks to them.</p>
<p>Despite my deepest resistance, I even got myself a walker, and we became best friends: I could go for walks alone again and sit down for a bit, and that gave me a sense of security. My sister and I were quite the sight when we were out and about—me with my glasses and walker, and my sister with her glasses—and then there was the resemblance!</p>
<p>I struggled on through my now considerably diminished life. My lung function test showed only 16-18 %. Despite this, I went to the gym and did pulmonary rehabilitation exercises, and I took my walks in the woods, which by then took forever – none of it was fun anymore, but giving up wasn&#039;t an option. Besides, you&#039;re supposed to be in reasonably stable health with the best possible fitness – or what&#039;s left of it – to get a transplant.</p>
<p>In January 2023, my sister and I received training and instruction on home self-therapy with Respreeza through the Thorax Clinic. This saved us a considerable amount of time in our daily lives. It&#039;s no longer relevant for me, but my sister still benefits from it.</p>
<h2>My lung transplant and how I felt afterwards</h2>
<p>After four years and two months, at 9:30 p.m., the long-awaited call came from the transplant center informing me that a lung was available. My two daughters were visiting for a week at the time. At first, I was literally speechless with shock, and then I became afraid. My daughters quickly packed my essentials, and an hour later, the ambulance picked me up and took me to Munich. My daughters and I had to say goodbye, fearing that we might never see each other again; but thankfully, that happens very rarely.</p>
<p>I actually managed to calm myself with the thought that I now had to place my life in the hands of the doctors, fate, or God, because I no longer had any control over it myself. Once I arrived in Munich, I still had to wait for the final go-ahead as to whether the new lung could actually be transplanted; until then, that wasn&#039;t certain.</p>
<p>At 6:00 a.m. I went to the anesthesia preparation room and was incredibly relieved when I was energized. Three days later, I was brought out of the medically induced coma and was happy to see my daughter at my bedside first. So everything had gone well and I was alive. And the breathing tube was out. I&#039;d been pretty scared of that, too.</p>
<p>I felt awful, even though everything had gone perfectly for me. I was in incredible pain and so weak I could barely speak. I felt like I wasn&#039;t breathing any better than before; only later did I learn that this is perfectly normal – if only someone had told me that beforehand!</p>
<p>During the first few days and sleepless nights, I seriously asked myself: What have I gotten myself into? The first few weeks were truly awful! After a week, I was moved to a regular ward.</p>
<p>My physiotherapist came every day, getting me back on my feet and moving again with ever new exercises.</p>
<p>We did breathing exercises daily because I had to relearn and practice proper breathing. My muscles had completely atrophied, and I had to fight my way back to life and onto my feet – still in considerable pain.</p>
<p>After five weeks in the hospital, I went directly to rehabilitation at the Schön Clinic in Schönau am Königssee. By then, I was feeling considerably better, and my fighting spirit had returned. In addition to training and medical care, we received comprehensive instruction and were prepared for life at home. Due to my suppressed immune system, there were several things for me and my family to consider – such as a low-germ diet, hygiene measures, and personal protection with masks, etc. Exchanging experiences with other transplant patients also made me feel more confident in dealing with my new situation. Meeting other people, some of whom had been transplanted decades earlier and were doing very well, gave me courage and confidence that I could manage and that everything would be alright. After a total of ten weeks, I finally went home. Initially, I was still very unsure of myself at home, but that quickly subsided, and I settled into a new daily routine. I am still incredibly happy about my new life. It was, and still is, an exhilarating feeling to breathe freely and get enough air again.</p>

	<p>I was able to accept them without any problems because he or she hadn&#039;t died for me. I am also incredibly happy and grateful for the support of everyone who treated, cared for, and accompanied me on my journey into my new life.</p>
<p>I&#039;m now a completely new person with an outstanding quality of life. I&#039;m delighted with my life every single day, and my loved ones share in my joy. I diligently train for fitness and lung health. I enjoy taking long walks through the Odenwald – my new home – and I find it incredibly rewarding. I can once again attend events, festivals, and concerts, meet up with friends, and do everything I feel like doing, things that were no longer possible.</p>

<p style="font-weight: 400;">Whether it&#039;s a cold, bronchiectasis, or the effects of Covid-19, many people are familiar with the problem of struggling with mucus in the body, and especially in the lungs.</p>
<p style="font-weight: 400;">Our pulmonary exercise trainer, Peter Kukry (Peddar), himself an Alpha-1 lung disease patient, therefore set to work and recorded the following video for us. He is supported by Dr. Heike Isensee, an active participant in the pulmonary exercise program and group leader of the Stuttgart self-help group. The video is rounded out with expert advice from Dr. Alexander Rupp of the Im Zentrum Stuttgart pulmonology practice.</p>
<p style="font-weight: 400;">Sometimes simple techniques and exercises can make everyday life significantly easier, try them out!</p>

	<p>First off – I think this device is fantastic. Weighing only 100 grams and practically silent, it&#039;s perfect for a quick inhalation on the go. It&#039;s easy to hold, doesn&#039;t disturb the peace even if other people are in the room or you have the TV or music on, is great for taking on vacation, and all the accessories are neatly organized in the handy little case (included). The device costs €106 including shipping and is covered by prescription. My private health insurance also covered the cost without any problems.</p>
<p>The nebulizer is charged via the included USB cable, and a single charge lasts for approximately one hour of inhalation time. So far, I&#039;ve only nebulized saline solutions (0.9 µT or 3 µT). According to the manufacturer, the particle size is less than 2 micrometers (µm), allowing it to effectively reach the lower respiratory tract. And it produces a good amount of mist... One minor drawback, in my opinion, is that some liquid tends to collect in the mouthpiece towards the end of the nebulization. This should be emptied. If you&#039;re not careful, it can easily spill and end up with damp clothes. Perhaps I&#039;m doing something wrong? After inhalation, I finish by nebulizing still mineral water (low in calcium) – I suspect distilled water would be better, but I don&#039;t have any on hand. My device hasn&#039;t developed any limescale buildup, and the membrane isn&#039;t clogged with salt. According to other users, descaling the membrane with vinegar is quite easy. Incidentally, the staff at Air-be-c (the company through which the device can be purchased) are extremely helpful and available by phone for any problems.</p>
<p>Of course, the SERVOX doesn&#039;t have special functions like the Pari Boy. It simply nebulizes medication continuously until the reservoir is empty. Breathing against resistance, combinations with devices like the RC Cornet, etc., are not possible. For the training effect or additional secretion mobilization, I then have to set up the Pari Boy.</p>
<p>I&#039;ve really neglected my Pari Boy since getting the SERVOX (although I haven&#039;t had an infection in the meantime). The Pari Boy is just so noisy and not exactly portable, so I rarely took it with me on shorter trips, whereas the SERVOX is easy to slip into my travel bag. Setting up and using the Pari Boy isn&#039;t something I do spontaneously; I only do it when I&#039;m really congested. I use the SERVOX much more frequently, almost as a precaution.</p>

	<p>Conclusion: Both devices have their place. I will continue to use the Pari Boy, especially when an infection is coming on or has already started, when I have a lot of mucus, and when I&#039;m at home. The SERVOX is a quick preventative inhaler for in-between times and a good alternative when I&#039;m out and about.</p>

	<p>My name is Christine Stukan, I am 58 years old and have two daughters, aged 25 and 22. I am now fully retired and have had a lung transplant for almost exactly a year. I am feeling better than I have in decades, so I am happy to be able to tell you my story today.</p>
<p>I have experienced almost all facets and stages of COPD – from diagnosis to lung transplant. I want to encourage you and give you confidence that perseverance and fighting on will always be worthwhile; for us and for our loved ones.</p>
<p>In the early 2000s, I was referred to a pulmonologist by my family doctor because I was experiencing significantly more difficulty breathing and shortness of breath than usual. It took another six and a half years until, in 2009, I received the diagnosis of AAT genotype PiZZ and began Prolastin replacement therapy. For this, I was admitted to the University Hospital in Munich-Großhadern. I shared a room with another Alpha1 patient who was there on the lung transplant waiting list. Up until then, I had rather naively thought that I was missing an enzyme and that once I received the replacement, everything would be fine. Through my fellow patient&#039;s story, I was forced to grasp the full extent of the potential consequences of the disease.</p>
<p>Over the years, my lungs deteriorated progressively. My daily life, juggling work, two school-aged children, and household chores, became increasingly difficult. I worked in an office, which is why it was still manageable. In 2011, as part of a rehabilitation program, I was prescribed long-term oxygen therapy. My Spirit (portable oxygen concentrator) and I became inseparable. With oxygen, I was actually able to exert myself considerably more. As a result, I applied for a higher degree of disability and received the additional designation &quot;aG&quot; (extraordinary walking disability), which entitles me to the blue parking permit for severely disabled individuals – a significant improvement in my daily life.</p>
<p>I struggled through life, which became increasingly difficult. Then, on top of everything else, I also had to organize and care for our elderly parents. I was barely managing to make ends meet with all this, but I had to, and my job was very important to me and provided me with stability. Due to my now dire health situation, the topic of transplantation as a possible option came back into focus.</p>

	<p>In 2013, I therefore went to rehab. I wanted to delve deeply into the topic of transplantation. Since it involves a highly invasive procedure with considerable risks, it was a very difficult decision with far-reaching consequences for me and my family. After careful consideration, however, I decided against it for the time being because of the risks and because my teenage daughters still needed me.</p>
<p>In 2018, I had four valves implanted in the lower lobe of my left lung. These worked exceptionally well, and my lung function improved significantly. For a few weeks, I had a completely new quality of life and could breathe properly again. Unfortunately, this was short-lived because the elasticity of my lungs decreased, which apparently can happen in rare cases.</p>
<p>This sublime feeling of being able to breathe properly again and get enough air finally convinced me to get listed for lung transplantation. I wanted to experience that quality of life again if I had the chance – despite all the risks.</p>
<p>In November, I spent two weeks at the university hospital in Munich-Großhadern for a pre-employment examination. This meant a complete and thorough check-up. Given my already very poor health, this was really stressful and exhausting.</p>
<p>My current state of health was assessed, and cancer was ruled out. A psychologist also evaluated whether I was mentally stable enough for this major and stressful surgery. She wanted to know if I would be able to adequately and sustainably care for myself and a new lung, and whether I could accept and appreciate a new organ. The consultations with the surgical and anesthesiology teams also took place within this framework, so that they could react quickly if a donor organ were found. Thus, in February 2019, I was placed on the transplant list, and it was a matter of waiting for an organ. My mental health deteriorated, and to make matters worse, I developed anxiety and panic attacks, which had a truly dramatic impact. I had up to ten panic attacks a day, which left me so incapacitated that I could barely breathe. It was so bad for me that I sought help at the respiratory emergency clinic.</p>
<p>It&#039;s part of the palliative care unit at the university hospital. They work with a completely different approach there: the shortness of breath originates in the lungs, but is triggered in the pain center of the brain. Low doses of morphine can improve the breathing somewhat. I also received help there through conversations with a psychologist, who subsequently recommended cognitive behavioral therapy.</p>
<p>In 2020/2021, I therefore began therapy to regain control over my psychologically distressing situation and to get the very debilitating anxiety and panic attacks under control. However, this only worked to a limited extent, since there was a physical cause for them. At least I learned to regulate myself in such situations so that I could breathe again and get enough air.</p>
<p>My health deteriorated steadily. As a result, I became increasingly fragile and hardly dared to do anything. Even the slightest exertion, like putting on socks or shoes, left me acutely short of breath. A cold or any other infection was a complete disaster for me. I could barely get through the day, even though I had nothing else to do but take care of myself. During this time, my daughter, who still lived at home, provided me with unwavering support, for which I am incredibly grateful.</p>
<p>During a rehabilitation program in 2021, I had to realize that I had to end my beloved job and could no longer live alone. This was a very difficult realization for me, as I had so cherished my independent life. I then applied for care level 2 through the social services department there and subsequently submitted an application for a disability pension after the rehabilitation program.</p>
<p>In the spring of 2022, I moved from Munich to Weinheim to live with my sister and brother-in-law. They had made me the incredibly generous offer to live with them, even though my sister herself was seriously ill. My heartfelt thanks to them.</p>
<p>Despite my deepest resistance, I even got myself a walker, and we became best friends: I could go for walks alone again and sit down for a bit, and that gave me a sense of security. My sister and I were quite the sight when we were out and about—me with my glasses and walker, and my sister with her glasses—and then there was the resemblance!</p>

	<p>I struggled on through my now considerably diminished life. My lung function test showed only 16-18 %. Despite this, I went to the gym and did pulmonary rehabilitation exercises, and I took my walks in the woods, which by then took forever – none of it was fun anymore, but giving up wasn&#039;t an option. Besides, you&#039;re supposed to be in reasonably stable health with the best possible fitness – or what&#039;s left of it – to get a transplant.</p>
<p>In January 2023, my sister and I received training and instruction on home self-therapy with Respreeza through the Thorax Clinic. This saved us a considerable amount of time in our daily lives. It&#039;s no longer relevant for me, but my sister still benefits from it. After four years and two months, at 9:30 p.m., the long-awaited call came from the transplant center informing me that a lung was available. My two daughters were visiting for a week at the time. At first, I was literally speechless with shock, and then I became frightened. My daughters quickly packed the essentials for me, and an hour later, the ambulance picked me up and took me to Munich. My daughters and I had to say goodbye to each other with the fear that, in the worst-case scenario, we might never see each other again; but thankfully, that happens very rarely.</p>
<p>I actually managed to calm myself with the thought that I now had to place my life in the hands of the doctors, fate, or God, because I no longer had any control over it myself. Once I arrived in Munich, I still had to wait for the final go-ahead as to whether the new lung could actually be transplanted; until then, that wasn&#039;t certain.</p>
<p>At 6:00 a.m. I went to the anesthesia preparation room and was incredibly relieved when I was energized. Three days later, I was brought out of the medically induced coma and was happy to see my daughter at my bedside first. So everything had gone well and I was alive. And the breathing tube was out. I&#039;d been pretty scared of that, too.</p>
<p>I felt awful, even though everything had gone perfectly for me. I was in incredible pain and so weak I could barely speak. I felt like I wasn&#039;t breathing any better than before; only later did I learn that this is perfectly normal – if only someone had told me that beforehand!</p>
<p>During the first few days and sleepless nights, I seriously asked myself: What have I gotten myself into? The first few weeks were truly awful! After a week, I was moved to a regular ward.</p>
<p>My physiotherapist came every day, getting me back on my feet and moving again with ever new exercises.</p>
<p>We did breathing exercises daily because I had to relearn and practice proper breathing. My muscles had completely atrophied, and I had to fight my way back to life and onto my feet – still in considerable pain.</p>

	<p>After five weeks in the hospital, I went directly to rehabilitation at the Schön Clinic in Schönau am Königssee. By then, I was feeling considerably better, and my fighting spirit had returned. In addition to training and medical care, we received comprehensive instruction and were prepared for life at home. Due to my suppressed immune system, there were several things for me and my family to consider – such as a low-germ diet, hygiene measures, and personal protection with masks, etc. Exchanging experiences with other transplant patients also made me feel more confident in dealing with my new situation. Meeting other people, some of whom had been transplanted decades earlier and were doing very well, gave me courage and confidence that I could manage and that everything would be alright. After a total of ten weeks, I finally went home. Initially, I was still very unsure of myself at home, but that quickly subsided, and I settled into a new daily routine. I am still incredibly happy about my new life. It was, and still is, an exhilarating feeling to breathe freely and get enough air again.</p>
<p>I am eternally grateful to my donor for giving me a new lung – and with it, a completely new life. I was able to accept it without any problems because he or she hadn&#039;t died for me. I am also incredibly happy and grateful for the support of everyone who treated, cared for, and accompanied me on my journey to this new life.</p>
<p>I&#039;m now a completely new person with an outstanding quality of life. I&#039;m delighted with my life every single day, and my loved ones share in my joy. I diligently train for fitness and lung health. I enjoy taking long walks through the Odenwald – my new home – and I find it incredibly rewarding. I can once again attend events, festivals, and concerts, meet up with friends, and do everything I feel like doing that was no longer possible.</p>
<p>Even if my health should decline again at some point, I will be grateful for this wonderful time I was given. I would choose to have a transplant again in a heartbeat. If you have any further questions, please feel free to ask!</p>

	<p>I already reported in the last journal about my troubles with the German Federal Pension Insurance Fund, from the application process to the approval of my rehabilitation at the correct facility. Shortly before the editorial deadline, I received an invitation to the Baltic Sea Clinic in Schönberg-Holm (OKLI) for the end of November 2023.</p>
<p>On my doctor&#039;s advice, I should avoid visiting such facilities during the 2023/24 flu season to prevent exacerbations. The facility understood this perfectly, and so we discussed postponing the start of my rehabilitation until shortly after Easter. Since the rehabilitation could begin more than six months after the initial decision, the German Pension Insurance (DRV) had to approve the new date. I contacted the same employee I had been in contact with regarding my &quot;lost files at the DRV.&quot; This went smoothly and quickly.</p>
<p>So, 13 months after the application was submitted, all the requirements for rehabilitation were met.</p>
<h2><span style="color: #004267;">Why Ostseeklinik Schönberg-Holm? </span></h2>
<p>There were several reasons for me to apply for my rehabilitation at OKLI:</p>
<ol>
<li>Rehabilitation in the mountains is beautiful to look at, but the inclines are almost impossible for me to manage. The harsher climate of the North Sea also doesn&#039;t agree with me. So I started looking towards the Baltic Sea.</li>
<li>During a previous rehabilitation stay in southern Bavaria, I learned what the right sporting activities are for me, as someone with Alpha-1 emphysema, to keep me as fit as possible. Since then, I&#039;ve been doing regular rehabilitation exercises, so even in a pulmonary rehabilitation facility not specializing in Alpha-1, I can articulate what I need and what&#039;s good for me.</li>
<li>My main diagnostic goal during rehabilitation was to determine whether I might already require supplemental oxygen or not.</li>
<li>I administer my own AAT substitution and always have everything I need with me. Therefore, there was no discussion about whether and how the supply could be provided at the rehabilitation facility. I had clarified this with the facility beforehand.</li>
<li>Finally, as an Alpha1 board member, I wanted to find out if there was a rehabilitation clinic in the north, on the Baltic Sea coast, where we Alphas would be well cared for. It couldn&#039;t be that it was always concentrated in the south!</li>
</ol>
<h2><span style="color: #004267;">So, what was it like at OKLI?</span></h2>
<p>I travelled here in my own car, also to take my bicycle with me.</p>
<p>The welcome at the reception desk was very friendly and warm. It made me feel like I was in a hotel. I was assigned a personal first aider who showed me around my room, explained the mailboxes, the clinic&#039;s wayfinding system, and the meal plan, giving me a good initial orientation. As soon as I was in my room, I went for my initial examination, followed by a consultation with the doctor. It was very thorough! They discussed my current status with my condition and my goals for this rehabilitation program. All of this culminated in my individualized therapy plan, which was then stored in the system and accessible to all doctors and therapists.</p>
<p>All rooms had either a small terrace or a balcony. A television on the wall, a chair, an armchair, a bed, a large shower room, and plenty of closet space made me look forward to the coming weeks.</p>

	<p>The restaurant truly lived up to its name. A generous breakfast and dinner buffet, along with a lunch menu served at the table, restored a distinct hotel atmosphere. Guests were even welcome to fill their own thermos mugs with coffee.</p>
<p>Therapy planning at OKLI was similar to that in other rehabilitation facilities. There was a mailbox where appointments and treatments for the following day were listed each evening. Conversations with other patients made it clear that everyone seemed to have their own individual plan.</p>
<p>In addition to the treatments, sufficient rest and relaxation periods were also ensured. At the beginning of rehab, it can seem as if too little is happening. This is probably necessary to allow you to unwind from your daily routine. In the second and third weeks, I then had a significantly more intensive therapy program.</p>
<p>Those who wanted to do more than their prescribed therapies could take advantage of the free time in the evenings at the gym and swimming pool. The location near the Baltic Sea, with the beach just 700 meters away, made for pleasant and relaxing walks. Well-maintained bike paths along and on the dike allowed me to enjoy longer bike rides with sea views. During my stay, guided bike tours were offered on weekends, providing information and insights into the Probstei region northeast of Kiel – not just for e-bikers. Highly recommended! Bicycles could be rented on-site. For those with their own bikes, there was a large shelter with electrical outlets for chargers.</p>

	<h2>My conclusion after three weeks</h2>
<p>Those who are already familiar with their condition and their AATD (acute and progressive hypertension) and primarily want to get fit for the coming period through physical activity or recover from recent exertion will find this facility a good fit. Depending on their prior knowledge, they can also attend the usual theoretical and background events on COPD, social services, severe disability, pension issues, etc. Newly diagnosed individuals with AATD who may require further diagnostic testing or therapeutic adjustments should consult their primary care physician beforehand to carefully determine whether all necessary measures are possible at the OKLI (Open Clinical Rehabilitation Center). This, however, applies to all rehabilitation programs.</p>

	<h2><span style="color: #004267;">One more thought on substitution: </span></h2>
<p>I keep hearing in our organization that facilities involved in substitution therapy try to avoid administering these procedures whenever possible. Sometimes, the reason given for refusal is that it&#039;s a blood transfusion, for which they aren&#039;t equipped. In my opinion, that&#039;s simply not true!</p>
<p>AAT is a medication administered intravenously. Any rehabilitation facility that draws blood can also administer it intravenously.</p>
<p>Anyone wishing to avoid this discussion could consider bringing their own medication for the planned rehabilitation period and discuss it with their family doctor. Then, only the administration of the medication would need to be coordinated.</p>
